Assessment Coordinator
The Assessment Coordinator is responsible for coordinating and completing all individualized assessments for youth referred to and enrolled in Wraparound services at Ascent's Baton Rouge location, serving W.
Feliciana, E. Feliciana, Pointe Coupee, E. Baton Rouge, W. Baton Rouge, Iberville, and Ascension parishes, and throughout other parishes via teleconference as needed.
This position will ensure that all assessments are submitted to the Statewide Management Organization (SMO). The Clinical Director supervises this position.
Primary Responsibilities
- Complete and maintain CANS certification
- Complete and submit assessments to the SMO in a timely manner
- Complete provider-focused Satisfaction Surveys during each reassessment
- Perform special projects and duties as assigned
- Maintain confidentiality in all aspects of client, (i.e. HIPPA, FERPA, P.L. 46 : 56) staff, and agency information
Qualifications
- Mental health professional licensed for independent practice (LMHP, LCSW, LPC, LMFT), eligible to render services via telehealth in accordance with their licensing board
- Master's degree in a human service-related field
- Ability to provide direct face-to-face communication with youth and families in their homes
- Must possess and maintain a current, valid Driver License
- Must be able to remain in a stationary position for extended periods while driving
Skill Requirements
- Strong time management skills and ability to meet documentation deadlines
- Professionally skilled in verbal and written communication
- Meticulously organized and detail-oriented
- Computer skills, including proficiency in MS Word, Excel, and Outlook
