Job Title:
504 Case Manager/Student Liaison
1-year contracted position
Part-time: up to 29 hours per week
$25 - $30 depending on certification level
Overview:
We are seeking an individual to fulfill the role of 504 Case Manager/Student Liaison. This position requires a unique blend of skills with 80-90% of time being spent managing 504 plans for students with disabilities with the remaining time spent supporting school counselors and school social workers in delivering comprehensive mental health services to students. The ideal candidate will be detail oriented, compassionate, with a high degree of skill using various types of computer software programs.
Responsibilities:
504 Plan Management:
Develop, review, and manage 504 plans for students with disabilities in accordance with state and federal regulations.
Manages 504 records within ECATS and student information management system data reports
Collaborate with parents, teachers, and other stakeholders to assess student needs and develop appropriate accommodations and modifications.
Monitor the implementation of 504 plans across different academic and extracurricular settings.
Ensure documentation of accommodations, progress, and evaluations are maintained accurately and confidentially.
Supports teachers with classroom management and behavioral strategies
Assists with behavioral and academic data collection
Notifies Principal, Director, and Program Specialist of students concerns as needed
Monitors the implementation of the 504 through informal observations and feedback
Transfers compliant files to other buildings upon student transfer or at year end
Upload 504 documentation to College Board or other required platforms.
Student Support Services:
Provide direct support and advocacy for students with diverse learning requirements, including academic, behavioral, and emotional challenges.
Conduct assessments and evaluations to identify students' strengths, and areas of growth.
Collaborate with teachers, counselors, and outside agencies to coordinate additional support services and referrals for students as needed.
Collaboration and Professional Development:
Foster positive relationships with school staff, parents, and community partners to enhance support networks for students with disabilities and diverse learning needs.
Participate in multidisciplinary team meetings, parent conferences, and professional development opportunities related to special education and student support services.
Qualifications:
Bachelor's in Education, Psychology, School Counseling or a related field.
Experience working with students with disabilities and/or providing student support services in an educational setting.
Proficiency with web-based applications, Microsoft Office (particularly spreadsheet, database, and word processing), and student information systems with a strong orientation to detail.
Demonstrated organizational and planning skills, including proven expertise in follow-up and follow-through to completion within established time frames.
Experience with disability accommodation and knowledge of ADA and Section 504 of the Rehabilitation Act preferred.
Strong interpersonal skills and the ability to build rapport with students, families, and colleagues from diverse backgrounds.
Excellent communication, problem-solving, and organizational skills.
Ability to work independently as well as collaboratively within a team-oriented environment.
