Children’s Long Term Support (CLTS) Case Manager
The Ashland County Health & Human Services Department is currently taking applications for Children’s Long Term Support (CLTS) case manager. The CLTS case manager provides support and service coordination to children living with an intellectual/developmental disability, physical disability, and/or severe emotional disturbance and their family/caregivers, with the primary responsibility of promoting the child’s health, safety, and welfare in their home and community.
Qualifications:
- Minimum of a four-year degree from an accredited college in social work, sociology, psychology, criminal justice or other approved human services program.
- Minimum of one year experience providing social work to children and families.
- Preference (but not required) will be given to individuals with social work experience, and those who are certified as a social worker in the State of Wisconsin or eligible for certification within one (1) year.
- Valid Wisconsin driver’s license (available licensed driver), insurance and reliable transportation.
Starting salary $21.88 - $25.17/hr based on experience. Excellent benefits including a 4-day flexible work week (35 hours), 2 weeks of paid vacation after 90 days of employment, health/life/dental insurance, and Wisconsin State Retirement.
