As a Nurse Practitioner at Aspen Physician Group, you are responsible for handling patient appointments and performing routine examinations to ensure patient health. You will speak with patients about healthy lifestyle choices for their age, completing diagnostic tests to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain updated medical records. You will develop bonds with our clients helping them work through life's biggest transition. We see our patients in assisted living, independent living, nursing homes and their private homes. Aspen Physician Group is a thriving physician owned and managed primary and specialty care practice catering to adult and geriatric patients across the state overseeing 100's of patients. We see our patients in assisted living, independent living, nursing homes and their private homes. We specialize in the care of complex patients and strive to deliver expert, personalized care to each individual. Each clinician has a remote support team to assist with daily tasks (medical assistant/office support). Out in the field, you will work closely with Assisted Living, Home Health and Hospice staff in ensuring the health and safety of our patients. Job Description:

  • Training/Onboarding to ease you into the day to day responsibilities of a Home Health Nurse Practitioner.
  • On an average day you will visit 5-10 patients per day (We plan to have that number grow as word-of-mouth travels about our quality of care).
  • Administrative work is completed onsite or at your home office.
  • Perform a comprehensive physical examination to identify the normal and abnormal changes associated with health, injury and /or disease states.
  • Order, perform, and interpret diagnostic procedures and tests, as indicated by the history and physical examination, for acute and common primary care conditions.
  • Develop differential diagnoses on the basis of clinical and laboratory data.
  • Differentiate common primary care problems and other deviations from normal. Recognize emergency situations and initiate emergency care/triage.
  • Provide for general health needs of patients, including health promotion, maintenance, and restoration. Initiate health promotion and disease prevention strategies.
  • Provide individualized client teaching and counseling in respect to current identified problems, disease states and social/cultural situations.
  • Complies with medical records policies regarding the complete and accurate documentation of patient care. Initiates and maintains required records and legal documents.
  • Participates in quality improvement activities.
  • Enjoys geriatric patients and long-term relationships.
  • Perform other duties as assigned.

Job Requirements:

  • Licensure to practice as a Nurse Practitioner in the State of Utah.
  • CPR/BLS Certification
  • Current Utah driver’s license and automobile liability insurance
  • Ability to communicate effectively, both verbally and in writing
  • Respecting the patient’s privacy and maintaining confidentiality
  • Experience with Homecare Homebase or other EMR not required but a HUGE plus
