Your Impact

The Case Manager (CM) will work directly with the participants in Transitional Housing Placement Program (THPP) for foster youth, ages 16-18 years old. The Case Manager will provide constant support, mentoring and supervision to the residents during their designated employment shift and will also collaborate with the Program Manager to adhere to the Admission Agreement of the THPP participants. The CM will assist the participants in a strength based approach to implement independent living skills, (i.e., money management, job seeking, volunteering in the community) and obtaining their educational goals prior to being on their own. The CM will work with the individual youth to mentor and teach these life skills in the home setting and will work closely with the referred youth, team members, and County partners to develop and follow through on strength-based, individualized plans for helping youth meet needs across a wide variety of life areas.   

Your Skills and Duties

  • Participate in the Orientation with Participant and the THPP Program Manager to ensure that Participant understands all expectations of the program and their role. 
  • Review the THPP Handbook with the participant upon admission. 
  • Review participant’s Individualized Needs and Service Support Plan.  
  • Understand Community Care Licensing (CCL) THPP regulations. 
  • Work directly with the THPP Program Manager in developing and implanting the schedule for the ILS weekly groups. 
  • Responsible for maintaining current CCL documents to be posted in common area (i.e., weekly menus, religious services in the community). 
  • Responsible for maintaining current CCL documents for participants in the program. 
  • Attend monthly THPP Team Meetings with the participant, THPP Program Manager and County representative. 
  • Provide excellent documentation for attendance in ILS skill building participation for the participant and review regularly with the THPP Program Manager. 
  • Have strong conflict resolution management skills to mentor the youth during weekly residential house meetings 
  • Develop a good working relationship with the Participant’s individual support team which supports his/her goals in the plan. 
  • Have strong communication skills, and demonstrate good boundaries with the participants. 
  • Participate in meetings as scheduled by THPP Program Manager or other Aspiranet personnel of the THPP. 
  • Participate in trainings as designated by District Administrator and/or THPP Program Manager. 
  • Monitor Participant’s attendance at work, school, continuing education, adult education/college, or any combination of the above working to meet the identified goals established by the Participant and their support team.  Maintain daily reports to consult with THPP Program Manager. 
  • Participant in weekly meetings with the Participant to assess program compliance. 
  • Review of the Individualized Needs and Service Support Plan as updated to ensure participant is getting the support needed   
  • Develop a good collaboration rapport with the Independent Living Program (ILP) program personnel, monitor participants’ participation and attendance. 
  • Utilize the resources of the ILP program for outside recreational activities. 
  • Establish a professional rapport with the County vocational assistance programs as well as within the community sector to establish employment opportunities for the Participant in THPP. 
  • Provide as needed transportation for the Participant for ILP classes, court dates, medical, dental, therapy, attorney meetings or other programmatic, or county meetings. 
  • Provide transportation to the participant(s) during weekly food shopping activities to support their independent living skills. 
  • Establish natural consequences for Participant when out of program compliance. 
  • Work with the Program Case Manager in developing programs to enrich THPP. 
  • Represent self and Agency in a professional manner. 
  • Maintain client and case confidentiality per Agency standards. 
  • Support and model Aspiranet values. 
  • Demonstrate knowledge, sensitivity, and responsiveness to cultural differences. 
  • Maintain appropriate professional boundaries with staff, participants, and community partners. 
  • Rotate on-call phone per schedule. 

Education and Work Experience 

  • Bachelors of Arts or Bachelor of Science degree and two years of experience working with children and knowledge of independent living skills training and emancipation process for the County foster care system. 
  • High School diploma and four years of experience working with children and knowledge of independent living skills training and emancipation process for the County foster care system. 
  • Excellent verbal communication skills are required in both English and Farsi

Licenses and Certifications

  • Meets all state required conditions of employment as set forth by Community Care Licensing for a licensed Community Care Facility; i.e. Fingerprint and Child Abuse Index Clearance, TB/Health Physical, valid California Driver’s License and clean driving record (Motor Vehicle Report).
