ESSENTIAL FUNCTIONS
The Field Physical Therapist is an experienced professional who administers therapy to the patients on an intermittent basis in the home, nursing home or other settings. He/She independently assesses, plans, and delivers this care in accordance with the physician’s orders. Able to assess patient status and identify requirements relative to age specific needs.
EDUCATION/EXPERIENCE REQUIREMENTS
· One or more years in home health, preferred
· Have a valid PT license in the state of Arizona
· Oasis experience, preferred
· Must have a working knowledge of computers
· Current CPR certification
· Current TB test
· Current Finger print card
· Must have valid Driver’s License
· COVID vaccination card or qualifying exemption
REQUIRED COMPETENCIES – KNOWLEDGE, SKILLS, ABILITIES
· Evaluating, developing, implementing, documenting, supervising and revising plans of care for patients
· Coordinating patient care with other staff members and physicians
· Documenting patient assessment, needs and care
· Assisting with coordinating and teaching rehabilitative care programs to meet the needs of staff, patients, caregivers and the community
· Provides quality care according to governing and ccreditation bodies
· Ensures protection of confidential information that is stored, handled or maintained by the Agency per HIPAA regulations
· Submitting required documentation in a timely manner – within 48 hours from time of visit
· Knowledgeable of electronic record usage
· Maybe responsible for managing up to 2 PTAs

Physical effort involving above-average standing, sitting, walking, stooping, kneeling, and detail dexiterity.
