Case Manager MHP- DeKalb, IL ($1000 Sign on Bonus!)
Since 1961, The Association for Individual Development (AID) has served individuals with developmental, intellectual, physical and/or mental health challenges, those who have suffered a trauma, and those at risk. We are looking for a Case Manager – Mental Health Professional (MHP) who exemplifies that mission, and who wants to work for an organization that makes a difference. Are you the right fit?
What will you be doing?
- Developing and implementing individual programs to maximize the clients’ independent community living skills
- Completing daily documentation of mental health services provided to clients
- Working with a team of behavioral health service providers within the agency to ensure a positive client-centered approach
What will you bring to the table?
- Bachelor’s degree in human services or a related field (required)
- Experience working in social services (required)
- A valid Illinois driver’s license and automobile insurance

- Navigation of stairs
- Ability to lift 50 lbs. or more
- Ability to provide services in a client’s home
- The use of your personal vehicle or agency vehicles to transport clients
