POSITION SUMMARY:

We are currently recruiting for a Housing Case Manager. This full-time position pays $22.03 per hour and is located in Silver Spring MD.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

· Works closely with the Housing Opportunities for Persons with AIDS (HOPWA) Housing Coordinator, functioning as a housing and medical case manager.

· The housing and medical case manager meets regularly with prospective and assigned housing clients via tele visits, face-to-face and by telephone.

· The housing and medical case manager conducts the initial and annual HOPWA housing assessments and re-assessments, medical case management initial assessments and reassessments (for clients receiving medical case management), creates and maintains client paper records for housing clients and enters detailed client assessments/reassessments/notes and scans all client documentation in the NextGen electronic health record.

· The housing case manager’s duties include; performing housing assessments and re-assessments, performing medical case management assessments and reassessments, assisting clients with completing housing applications (when needed), reviewing housing applications for completeness, making and monitoring referrals to community partners for support services, interacting with landlords on client’s behalf to resolve housing issues, performing housing inspections/re-inspections, and assisting with creating quarterly reports.

· Documents all encounters with clients in the NextGen EHR accurately, timely and professionally.

· Meets with approved housing clients to complete application process and file setup.

· Updates all HOPWA required documentation and perform renewals and inspections as required.

· Performs high quality initial comprehensive assessments (for clients receiving medical case management) and reassessments and documents all encounters in the NextGen electronic health record system.

· Works closely with medical case managers and clinic staff regarding client’s barriers to care.

· Makes home visits/tele visits to clients as needed for inspections, etc.

· Works with clients to make referrals and or secure resources to community partners (child support enforcement, HOC, etc.) to increase housing stability.

· Ensures all adherence to HIPPA regulations.

· Participate in developing department goals, objectives, and systems.

· Participate in staff meetings and attend other meetings and seminars as assigned.

· Recommend new approaches, polices, and procedures, to continually improve efficiency of the department and services performed.

· Efficiently performs duties as assigned and works as a team within division and company.

· Perform other related duties as assigned.

Required Skills/Abilities:

· Experience and knowledge with HIV population preferred.

· Spanish or French language proficiency preferred.

· Ability to demonstrate interpersonal skills and ability to engage and work effectively with staff, client populations and medical service providers.

· Ability to demonstrate strong computer and data entry skills.

· Strong ability to meet deadlines.

· Excellent verbal, written, and listening communication skills within the English language.

· Proven, high quality customer service (internal and external customers).

· Ability to work collaboratively.

· Solid ability to analyze situations carefully and adopt appropriate courses of action.

· Ability to understand and adhere to the duties, methods, and procedures required by the position.

· Excellent organizational skills and attention to detail.

· Ability to prioritize tasks.

· Ability to effectively communicate to large and small group audiences.

· Proficient with Microsoft Office Suite or related software.

Education and Experience:

· BA in social work preferred

· Two years of experience in case management, housing preferred.

Physical Requirements:

· Prolonged periods of sitting at a desk and working on a computer.

· Must be able to lift and move up to 25 pounds at times.

· Standard hours of work Monday through Friday, 8:30 am – 5:00 pm, however evening and weekend hours may be required.

· Travel may be required up to 10% of the time.
