The Outpatient Nurse Practitioner (NP) or Physician Assistant (PA) will play a key role in providing high-quality, patient-centered care to individuals seeking primary care services at our client’s outpatient clinic. Working collaboratively with our multidisciplinary team, the NP/PA will assess, diagnose, and treat a diverse patient population, focusing on health promotion, disease prevention, and the management of acute and chronic medical conditions.
Key Responsibilities:
- Conduct comprehensive health assessments, including medical history review, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans.
- Develop and implement evidence-based care plans in collaboration with patients, incorporating their preferences, cultural beliefs, and individual needs.
- Prescribe medications, order diagnostic tests, and perform procedures within the scope of practice and in accordance with state regulations and organizational policies.
- Provide patient education and counseling on preventive health measures, lifestyle modifications, medication adherence, and self-management strategies for chronic conditions.
- Coordinate care with specialists, ancillary healthcare providers, and community resources to ensure continuity and effectiveness of care.
- Document patient encounters accurately and timely in the electronic health record (EHR) system, maintaining confidentiality and compliance with regulatory standards.
- Participate in quality improvement initiatives, clinical research projects, and staff education activities to enhance patient outcomes and promote professional development.
- Collaborate with other members of the healthcare team, including physicians, nurses, medical assistants, and administrative staff, to optimize workflow and patient satisfaction.
Responsibilities:
- Master's degree in Nursing (NP) or Physician Assistant Studies (PA).
- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of NY.
- Minimum of 1 year of clinical experience in primary care or a related field preferred.
- Proficiency in conducting comprehensive physical examinations, interpreting diagnostic tests, and formulating differential diagnoses.
- Strong interpersonal and communication skills, with the ability to establish rapport with patients and collaborate effectively with interdisciplinary team members.
- Commitment to providing culturally sensitive care and promoting health equity among diverse patient populations.
Please note- The annual base salary range is $100k to $135k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
