Senior Case Manager

Overview

We are seeking a dedicated Senior Case Management to join our team. The ideal candidate will have a strong background in case management and program management.

Responsibilities

• Lead and oversee the Extended Case Management (IAR, ICM, ECM and others) programs, including day-to-day operations, staffing, and case management activities.
• Supervise program staff, including onboarding, providing regular supervision, coaching, and professional development.
• Maintain up-to-date knowledge of ORR Preferred Community guidelines, CalAIM Enhanced Care Management, and Community Supports regulations, and others to ensure program compliance.
• Ensure programs and staff strictly adhere to HIPPA, PHI and PII requirements.
• Support the development and execution of short/ long-term program strategies.

Requirements

• MUST have a Master's in Social Work (MSW)
• Associate Clinical Social Worker (ASW) working toward licensure -OR- 2 years active unrestricted license in California as a Licensed Clinical Social Worker (LCSW) required with experience providing clinical supervision
• 2 years program administration experience
• Minimum of 4 years in Case Management, health education, or related fields
