What are the responsibilities and job description for the PHARMACIST - INFUSION position at Aultman Health Foundation?

PURPOSE OF POSITION

The purpose of the Infusion Pharmacist is to oversee the provision of specialized, complex pharmaceutical products; development and execution of plans to manage the medication therapy of patients; and clinical assessment and monitoring of patients in their homes. These services generally include home infusion therapy; other injectable drug therapy; parenteral and enteral nutrition therapy; and occasionally preparation of other sterile preparations, compounds or products.

RESPONSIBILITIES & EXPECTATIONS

  • Reviews and interprets all physician orders for processing.
  • Checks all processed orders and approves for delivery and release.
  • Clarifies medication, doses, route of administration and other related information with the physician.
  • Knowledgeable of age specific doses and routes of administration.
  • Knowledgeable of drug/drug interaction, food/drug interaction, compatibility and stability of intravenous medications. This includes chemotherapy medications and total parenteral nutrition.
  • Prepares and/or supervises the preparation of extemporaneously compounded medications, including sterile products adhering to USP 797 & USP 800 requirements.
  • Inputs patient and medication order information into pharmacy computer to facilitate medication dispensing.
  • Provides concise drug information to physicians, nurses, patients and other allied health professionals.
  • Knowledgeable of different drug information references and sources including websites.
  • Maintains control and security of all medications.
  • Supervises the activities of pharmacy support personnel related to the preparation and processing of orders and their performance.
  • Performs correct calculations of doses, infusion rates and compounded ingredients.
  • Accurate with dispensing of medications and entering orders into the pharmacy computer.
  • Understands different legal and regulatory requirements as well as Joint Commission standards.
  • Performs clinical interventions to clarify incorrect or inappropriate medication orders.
  • Performs clinical interventions to optimize medication therapy, patient safety, and cost effectiveness.
  • Stays current with current developments in the profession of pharmacy, new drugs and & current literature for drug therapies.
  • Assists in the training and development of new infusion employees.
  • Follows medication use policies and complies with drug formulary policies and procedures, including implementation of the P&T Committee approved therapeutic interchanges and non-formulary requests by prescription clarification with the provider.
  • Serves as a resource to patients, caregivers, referral sources and interdisciplinary staff members concerning medications, their interactions and side effects.
  • Consults and collaborates with the interdisciplinary team.
  • Reports any suspected abuse or specific threats of harm to self or another person.
  • Demonstrates use of age specific principles in communicating and educating population of service.
  • Consistently communicates with all patients, caregivers, customers and other personnel in a professional, compassionate, respectful and courteous manner.
  • Shows respect for and supports the patient's culture, language, customs and beliefs.
  • Demonstrates flexibility in meeting the needs of the department and responds positively to changes in workflow.
  • Promotes teamwork by demonstrating willingness to assist others in meeting the needs of all individuals and collaborating effectively with the interdisciplinary team and other departments throughout the organization.
  • Complies with agency infection control and safety policies.
  • Completes annual competency and licensure requirements.
  • Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel.
  • Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by Unit Director or Lead Pharmacist.
  • Consistently maintains a professional appearance.
  • Maintains Aultman standards of confidentiality, including the HIPAA Privacy Rule.
  • Participates in the Homecare and Infusion program's quality assessment/performance improvement process.
  • Participates in regular department meetings.
  • Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation.
  • Supports strategic organizational goals.
  • Abides by the Aultman Code of Conduct at all times.
 

 

Job Requirements


 

 

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (PharmD) from a college or a university accredited by the American Association of College of Pharmacy.
  • Has a current license as a Registered Pharmacist in the State of Ohio.
  • Excellent interpersonal and oral/written/telephone communication skills; strong organizational and problem-solving skills; ability to stimulate new ideas.
  • Valid Ohio Driver's License
  • Must have own transportation and provide proof of current liability insurance
  • Above average word processing and data entry skills. 
  • Applicable background checks must be passed.
  • Frequent interactions and communications with patients and patient’s family members is expected.
  • Prior experience in infusion pharmacy, retail pharmacy, nursing home pharmacy, or hospital pharmacy is preferred but not required.

WORKING CONDITIONS:

  • 8:00am – 5:00pm Monday to Friday or hours as required by the job including occasional overtime and rotating on-call coverage of evenings, weekends and holidays.
  • Subject to frequent interruptions and changes in priority of duties throughout the day.
  • Sitting/standing/moving about during working hours.
  • Hazardous Exposure Category 2
