Role Overview:
We are on the lookout for a compassionate, dedicated, and experienced Registered Nurse Care Manager (RNCM) to augment our healthcare team. Our ideal candidate is seasoned in navigating the complexities of chronic medical and behavioral health conditions prevalent within the geriatric population. Your expertise in managing a spectrum of diagnoses, coupled with addressing social determinants of health, will be pivotal in our collective success.
Key Responsibilities:
- Coordinate and implement personalized care plans for patients by employing clinical judgment and nursing processes.
- Engage in proactive outreach to initiate care planning, assess historical utilization drivers, and develop hospital avoidance strategies.
- Monitor patients' goal progression, ensuring adherence to appointments and mitigating avoidable ED or inpatient visits.
- Educate patients and caregivers, empowering them to manage health conditions actively.
Transitional Care Management:
- Conduct telephonic outreach to patients during and post-hospitalization to coordinate care and ensure seamless transitions.
- Complete medication reconciliations and foster educational conversations on health maintenance and disease management.
Care Team Communication:
- Present complex patient cases during interdisciplinary team huddles to identify interventions and recommendations for care.
- Collaborate with a wide range of healthcare professionals and community organizations to ensure comprehensive patient care and outcomes.
Supervision:
- Supervise Licensed Professional Nurses (LPN), guiding in-person visits for high-risk patients and supporting their care management efforts.
Documentation:
- Maintain accurate and timely patient records, ensuring documentation reflects care coordination efforts and complies with billing requirements.
Minimum Qualifications:
- Authorization to work in the U.S.
- Current and valid RN license.
- 5 years of RN experience, preferably in acute care or case management.
- In-depth knowledge of RN and LPN scope of practice, complex diagnoses, mental health and substance use disorders, and chronic disease management.
Skills and Experience:
- Proven experience with geriatric populations and complex/chronic medical conditions.
- Exceptional multitasking, critical thinking, and decision-making skills.
- Experience supervising LPN, focusing on chronic disease management and transitional care.
- Strong collaboration and relationship-building skills within team settings.
- Competency in utilizing technology (e.g., computer, smartphone, tablet applications) for patient engagement.
- Knowledgeable in addressing social determinants of health.
Preferred Qualifications:
- Bilingual in Spanish-English or Haitian Creole-English.
