Job Purpose

Our Administrative Care Coordinators perform reporting and administrative duties to facilitate the flow of all Avance Care locations and maintain quality care and Patient Centered Medical Home certification.

Essential Duties and Responsibilities

  • Performs Pre-Visit Planning for all providers on a daily basis
  • Involvement in Quality Initiative projects as assigned by manager
  • Oversee and perform all personal and automated recalls including, but not limited to, weekly follow-up recalls and CPE recalls
  • Following up with other clinics on outstanding diagnostic imaging
  • Managing fax inbox
  • Update the Patient Registry
  • Report uncontrolled chronic conditions to providers
  • Quality measure reporting
  • Patient follow-up care review
  • Provider operational dashboard of provider performance
  • Generating monthly follow-up recall letters
  • Website file drop uploading
  • Processing returned mail
  • Audit tasks performed by outsourced resources
  • Accomplishes all tasks as assigned or become necessary

Qualifications

  • High School Diploma, required
  • Associate degree in Healthcare Management, Business Administration, or similar field
  • 0-2 years relevant experience

Job Type: Full-time
