Job Title: CCM Manager
Reports to: Director of Care Management
Classification: Exempt, Salaried
Job Purpose: Maintain and support the growth of the Chronic Care Management service line, coach and assist direct reports in problem solving, and assist in new initiatives to improve operations and patient care regarding care management.
Essential Duties and Responsibilities
  • Oversees the following services:
  • Maintains full understanding of processes for CCM and all Avance Care service lines
  • Supervises all hybrid staff daily to ensure proper procedures are followed including but not limited to:
    • Addresses and resolves employee complaints and conflicts
    • Ensures all employees are following Avance Care Policies and Procedures and acting in a way that represents the company in the best light to patients
    • Maintains appropriate patient census, utilizes time efficiently
    • Completion of team responsibilities of ACO activities
  • Supports the administrative growth of service lines through employee performance including but not limited to:
    • Recruits, interviews, trains, counsels, coaches, disciplines, evaluates, delegates, documents, and terminates employees, as necessary, in collaboration with Clinical Manager
    • Schedules employees, oversees vacation requests, ensures continuous shift coverage, and approves payroll
  • Implements new tasks and protocols in collaboration with Director of Care Management
  • Addresses provider issues or concerns with department or other parties, as requested
  • Coordinate with other departments for resolution
  • Provide MA level clinical support for patient care as needed
  • Collaborate with Clinical Manager for end of month completion, clinical coverage due to staff outage
  • Communicates with Providers and location management as needed for support
Additional Duties and Responsibilities
  • Accomplishes all tasks as assigned or as they become necessary
Qualifications
  • Minimum of High School diploma or equivalent education required
  • Bachelor’s Degree in Business, Healthcare Administration, Management, or similar field preferred
  • 2-3 years relevant experience, including leadership

  • Detail-oriented
  • Strong time management and workload prioritization
  • Understanding of statistics and measuring performance-based metrics
Language Skills:
Ability to communicate well in writing and in person. Ability to read and interpret common medical terminology and abbreviations.
