ESSENTIAL POSITION FUNCTIONS:
A. Job Knowledge and Role Responsibilities
1. Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
2. Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, and infectious diseases if applicable.
3. Attends all mandatory inservices by the employee’s anniversary date.
4. Acts appropriately under the direction of the Director of Social Services and acts as an active member of the interdisciplinary and Quality Assurance team.
5. Demonstrates ability to adjust to changes in unit/shift assignments to meet resident and family needs.
6. Demonstrates knowledge of and complies with the Code of Conduct and Corporate Compliance Program when performing work functions.
B. Administrative/Resident Care Responsibilities:
1. Provides orientation to long term care facility and services for residents and family members.
2. Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement.
3. Schedules appointments with resident, responsible party, guardian, next of kin or sponsoring agency to:
a) Review and interpret Bill of Rights, including facility Policy and Procedure implementation. Obtain signatures, sign as a witness to signatures, and provide resident or above-mentioned representative with copies.
b) Obtain signature from resident authorizing release of medical records from referring agency or responsible party.
c) Obtain signature and sign as witness as per form of facility “Authorizing the Release of Hospital and Medical Records” when indicated.
d) Attend and lead Patient Care Conference Meetings within 21 days of resident admission.
4. Demonstrates responsibility for resident transfers to the following:
Hospitals:
a) Notifies all disciplines if resident is recipient of Medicaid funding if resident’s condition does not warrant a return within 15-20 day period.
Community:
a) Obtains current release of information.
b) Conducts a discharge planning conference at the discretion of the planner.
c) Issues pending discharge bulletin for resident.
d) Formulates post-discharge summary.
Intra-Facility Discharge:
a) Completes a discharge summary.
6. Attends Patient Care Conference meeting.
7. Performs various Quality Assurance audits.
8. Monitors departmental budget and reports expenses per facility policy to the Administrator.
9. Works with the Administrator in maintaining established activity standards.
C. Job Responsibilities-Documentation:
1. Completes Social History after conference with resident, family and/or responsible party within fourteen (14) days of admission.
2. Completes the initial psychosocial assessment section of Social History within fourteen (14) days of admission.
3. Completes Social Service Plan including Discharge Plan within fourteen (14) days of admission.
4. Ensures social service assessments are updated for residents returning from a 15-20 day hospital bed-hold, and for facility residents transferred within the facility at the same level of care.
5. Maintains significant social service progress notes on the resident’s medical chart on a timely basis and, at least quarterly, completes a progressive assessment note.
6. Maintains a current social service plan and discharge statement on the resident Care Plan.
7. Documents a progress notation regarding discharge/transfer and the reason for relocation, date, time, facility to which transferred and method of transportation and readiness intervention.
8. Maintains separate file on every resident to be filed in the Social Work office.
9. Completes advance directives.
D. Job Responsibilities - Personal Needs and Services:
1. Counsels residents regarding psychosocial programs.
2. Works directly with family or responsible party to enhance functioning of resident in the facility.
3. Assists with financial needs of resident, stressing confidentiality of records, and demonstrates knowledge of proper facility departments of Medicaid coverage.
E. Role Responsibilities - Interpersonal Skills:
1. Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information.
2. Interacts with residents and family members, co-workers, clinical and ancillary staff in non-judgmental, supportive and calm manner.
3. Addresses resident and family satisfaction issues immediately by reporting them to the Supervisor in a professional manner.
4. Is aware of the Resident Abuse Reporting Law.
5. Participates as assigned in the orientation of new staff and demonstrates a positive, helpful and enthusiastic attitude.
F. Role Responsibility - Safety:
1. Demonstrates ability to locate emergency equipment (i.e. fire extinguisher, oxygen equipment, etc.).
2. Understands and demonstrates knowledge of role in Fire and Disaster drills.
3. Demonstrates ability to identify and respond appropriately to potential behavioral outbursts.
4. Uses facility equipment safely.
5. Recognizes, removes, and/or reports potential hazards.
6. Follows established safety rules/accident prevention.
G. Role Responsibilities - Infection Control:
1. Demonstrates proper hand washing techniques
2. Disposes of waste properly.
H. Role Responsibilities - Resident Dignity:
1. Addresses residents in a respectful manner.
2. Discusses confidential resident information in appropriate areas only.
I. Other duties as assigned by Director of Social Services.
J. Physical Requirements:
“Occasionally” equals approximately 1% to 33%of the workday, “Frequently” equals approximately 34% to 66% of the workday, and “Continuously” equals approximately 67% to 100% of the workday (typical full-time workday is 7.5 hours).

SUMMARY OF QUALIFICATIONS:
1. Possesses a Master’s Degree from an accredited institution.
2. One to two years of experience in a related field preferred.
3. Possesses growth in the profession through community involvement and continuing education.
4. Explores and utilizes available community resources applicable to residential health care facility programs.
5. Must meet health assessment requirements, including two stage Mantoux skin test.
6. Must be capable of performing the essential functions of the job, with or without reasonable accommodation.
7. Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
Salary : $20 - $25
