Avera is currently seeking Nurse Practitioners (NP) looking to deliver the best possible nursing care in Avera’s patient-focused settings.

Job Summary
- Assumes the responsibility for the medical management of patients under the care of Avera@Home Hospice under the direction of the Hospice physician.
Education and/or Experience
- Graduate of an NLN accredited school of nursing or equivalent; graduate from an accredited Nurse Practitioner program required.
- Master’s or doctorate degree required.
- Experience in Hospice and/or Palliative Care as a CNP is preferred.
Licensure/Certification/Registration
- An active CNP license is required in Iowa upon starting.
- An active CNP license in Nebraska, South Dakota, and Minnesota may be required depending on the service area. If required, must obtain within 90 days of being placed in the position or of the requirement.
- Licensed as a Registered Nurse in the state of South Dakota and/or meeting the requirements of multi-state licensure is required.
- Licensure as an RN in the state of Minnesota may be required depending on the service area. If required, must obtain within 90 days of being placed in the position or of the requirement.
- Federal and State DEA certification is required for South Dakota and Iowa employees.
- Federal DEA for Certification only is required for Nebraska and Minnesota employees.
- National Board for Certification of Hospice and Palliative Care preferred.
- If Iowa licensure is required, Completion of the Child Abuse & Dependent Adult Abuse courses for Iowa Mandatory Reporters within 90 days of employment and every 3 years thereafter.
Work Schedule
- 8:00am-4:30pm; PRN, as needed
