Job requirements

  • Attending daily morning meetings.
  • Weekly Medicare meetings, leading, planning and completing all discharges.
  • Gathering discharge information from new skilled patients within 48 hours of admission.
  • Planning and attending discharge care plan meetings.
  • Addressing discharge concerns.
  • Documenting discharge notes in PCC.
  • Completing discharge audit forms.
  • Assist with annual post form audit.

We will also accept a licensed social worker for the position however, it is not a requirement.
