POSITION SUMMARY:
This Case Manager at Avivo will provide access to community support services for adults experiencing mental illness and addiction patients that are at greatest risk by developing a collaborative discharge and transitions programs with Allina hospitals to help client with stabilization. We provide short term, intense case management services that are innovative, person centered, strength based, and outcome driven. We strive for the highest quality of services using culturally responsive and evidence-based practices in a trauma informed environment. Clients referred to this service are at risk of requiring more restrictive care, or were recently discharged from a hospital setting, or have frequently been hospitalized or visited the emergency department and they have agreed to a referral for a case manager. The case manager works with clients recently admitted or seen at a hospital and high-risk patients living within the community and are living with serious mental illness and complex co-occurring disorder to develop recovery goals based on their individual needs.
Avivo's Chemical & Mental Health division provides integrated services in the areas of substance use disorder treatment and community-based chemical and mental health. We provide family residential treatment, co-occurring outpatient treatment for individuals and families, chemical use assessments, peer recovery support, mental health case management services, care coordination, and Community Support Programming. Services are innovative, person-centered, strength-based, and outcome-driven. We strive for the highest quality of services using culturally responsive and evidence-based practices in a trauma-informed environment.
ESSENTIAL RESPONSIBILITIES:
- Responsible for delivering behavioral health intakes and interim care coordination services. Services will begin when an eligible person is admitted to or seen at an Allina or other eligible hospital or referred by a PCP or other medical provider.
- Review the assessment of needs, establish a case plan designed to meet the immediate service needs and set up on-going services in the community.
- Coordinate physical and behavioral health management using the concept of Whole Person Care while addressing both client-center goals and health related social needs to efficiently improve overall health of the client.
- Assure that services provided, as developed by the client and case manager, are reflected in the Individual Community Support Plan. If clients’ goals change substantially, the case plan will be updated to reflect this.
- Coordinate services and supports necessary to assist each client as they work towards their goal of recovery.
- Assure that all changes in client status or needs are communicated through documentation.
- Assist in coordinating care with clients’ medical providers i.e., PCP, dentist, specialty providers and preventative care.
- To the extent requested by the client, provide collaboration and education to significant others and family members who are interest in assisting the client in their recovery process.
- Advocate on behalf of each client to assure the protection of client rights and privacy, as well as assure that clients understand the complaint and grievance procedures.
- Act in a manner to assure client confidentiality and privacy that complies with MHR policies and procedures/HIPAA/MN Medical Records Act.
- Treat all clients with dignity and respect, providing services that are respectful of age, gender, and culture.
- Connect clients to necessary community agencies to ensure closure in a timely manner.
- Participate in clinical supervision, staff meetings, and meet regularly with program coordinator for individual supervision to meet supervision requirements.
- Actively participate in agency’s undoing racism plan.
QUALIFICATIONS:
Required:
- Bachelor’s Degree in behavioral sciences or related field required, including but not limited to social work or psychology from an accredited college or university; or one year of experience in case management.
- Experience with and/or a demonstrated commitment to developing and delivering culturally responsive services.
Desired:
- Two years case management experience of which at least one year is in community based mental health with adults recovering from mental illness or combination of equivalent experience and training.
- Previous work experience in a behavioral health setting.
- Demonstrated understanding of the recovery philosophy and recovery oriented behavioral health services, or willingness to learn recovery principles.
- Ability to work independently with participants in home or community settings.
- Able to maintain positive presence in a fast paced, demanding, multi service environment, while remaining flexible and adaptable to changing service needs.
- Availability to work outside of normal business hours in order to meet the needs of the participants.
- Licensed or dually licensed professionals encouraged to apply (mental health and chemical health.)

WAGES: $19.83 - $24.79 per hour, Depending on Qualifications
