Description

Avivo's Ending Homelessness division provides pathways to housing for individuals experiencing homelessness. We work with county coordinated entry systems and tribal governments, as well as operating a unique street outreach teams to connect unsheltered individuals with shelter and housing. We assist individuals as they find and maintain housing, supporting them in scattered-site apartments in the community as well as site-based housing. We provide transitional recovery housing for individuals and families participating in Avivo's treatment programs. We operate Avivo Village to address the needs of individuals experiencing unsheltered homelessness. Avivo Village is a low barrier and culturally responsive shelter with individual “homes” focused on moving residents to permanent housing. Avivo’s Ending Homelessness Division strives for the highest quality of services using culturally responsive approaches and a deep commitment to harm reduction and housing first through trauma informed service delivery.

 

The Senior Case Manager position supports individuals and/or families in scattered house programs (Hotel to Home, Red Lake, Housing Support Hennepin, Metro, Women’s Housing Partnership, Housing and Urban Development, Collaboration of Housing Resources). The Senior Case Manager, Housing/Mental Health (TCM) will provide Rule 79 Adult Mental Health Targeted Case Management. The position will work with a diverse range of individuals experiencing serious and persistent mental illness providing mental health stabilization through assessment, linking, follow-up, and monitoring. The Senior Case Manager works with clients while they are in shelter to assist with linking to community resources, referrals and coordination to medical, mental health, and chemical health services, obtaining identification documentation, and education regarding accessing resources. The overall goal of the program will be to assist clients in utilizing their time, while in shelter, while looking for housing opportunities, to complete tasks which will ensure greater success once housed. The Senior Case Manager will be working in an unpredictable work environment and is highly skilled in meeting people where they are at, while delivering the highest quality services using culturally responsive and evidence-based practices in a trauma-informed environment.

 

PRIMARY RESPONSIBILITIES:

 

  • Provide direct client services:
  • Expert on internal and external culturally appropriate mental health resources.
  • Develop and implement assertive and progressive engagement strategies.
  • Build meaningful and purposeful client relationships with appropriate boundaries.
  • Able to inform and educate others on eligibility for programs and services.
  • Make appropriate referrals internally and externally.
  • Complete 3 or more creative attempts per client if needed.
  • Use creative strategies to engage in goal planning.
  • Engage and build rapport with clients.
  • Assess eligibility for program and services.
  • Connect individuals to desired and necessary services.
  • Develop Goal Plan.
  • Consult regularly with supervisor about client situations.

Qualifications


REQUIREMENTS

 

  • Bachelor's Degree in a Human Services related field.
  • 2,000 hours of supervised experience working with adults with mental illness.
  • Ability to provide Rule 79 Adult Mental Health Targeted Case Management per DHS guidelines.
  • Three or more years of experience working with single adults experiencing homelessness or similar experience.
  • Lived experience will be valued and considered as part of the experience requirement.

 

PREFERRED

 

  • Expert knowledge and awareness of mental health (specifically SPMI), chemical health, community resources, trauma, institutional racism, gender issues, tenant’s rights
  • Ability to train team members and actively engage in Harm Reduction and Housing First practices.
  • Train team members and engage assertive and progressive engagement modalities.
  • Train team members and engage in person centered care and language.
  • Support team members in identifying and creating healthy boundaries.
  • Train team members in Microsoft Suite and additional internal systems
