- Starting pay is $16.50 and ranges up to $18.49 per hour which considers preferred experience and preferred education described below.

Job Summary:
- Responsible for outreach activities within Montezuma County: Visits Community partner locations to offer services of Axis Health System (AHS) to potential patients; initiates the patient enrollment process, provides limited assessment capacity to guide enrollment into the appropriate services and conducts warm hand-offs with clinical staff to ensure continuity of care; works directly with the Engagement Services Manager to deliver a consistent outreach message. This position also facilitates enrollment in a Qualified Health Plan through the Marketplace or public assistance programs such as Medicaid for those requiring assistance. Responds to Social Determinants of Health (SDoH) screenings in the integrated clinic
Supervisory responsibilities: No
Physical Demands:
Employee must occasionally lift and/or move up to 15 pounds. Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. AHS may make reasonable accommodations to enable individuals with disabilities to perform the essential functions.
Physical Abilities:
Vision must be sufficient to read medical records, instruments, documentation, and visual changes in patient condition. Hearing must be sufficient to interact with a patient who may be unable to speak at a normal voice level. Mobility (including lifting, bending, stooping, reaching) and dexterity sufficient to assist patients and handle required instruments.
Travel required: Occasional
Required Education and Experience:
- Education: Bachelor's degree
- Minimum Experience: Previous experience with high-needs patients, case management and interagency collaboration.
- Must be knowledgeable regarding patient health information and HIPAA compliance.
- Must be organized and demonstrate the ability to follow guidelines.
- Must demonstrate excellent interpersonal and communication skills.
- Must demonstrate the ability to be self-directed.
- Must demonstrate the ability to follow the organization’s confidentiality and security rules.
Required Skills:
- Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax, and multi-line telephone.
- Working knowledge of Windows based computer applications including MS Word, MS Excel, MS Outlook, and ability to access the internet.
- Ability to work independently with minimal supervision and plan own work schedule effectively; multi-task and prioritize in a fast pace setting.
- Ability to demonstrate flexibility and responsiveness to unexpected clinic, patient, and program needs
- Ability to communicate and implement administrative, clinical, and procedural standards
- Ability to establish and maintain effective working relationships with staff and management, as well as with outside agencies, vendors, and patients in a professional, courteous, and confidential manner
- Ability to communicate program needs clearly and effectively program needs and services to individuals and groups outside the organization
- Knowledge or ability to learn Care Coordination Models and Community Outreach Programs
- Ability to learn new patient care documentation systems
- Ability to communicate effectively both orally and in writing
- Ability to read and understand written materials.
- Must regard work as confidential.
- Must have current driver’s license, qualify for commercial vehicle insurance, and maintain a driving record that allows insurability with AHS’ insurance agency.
- Occasional travel required
- Ability to take on additional duties when necessary.
- Annual recertification with Connect for Health Colorado as a Certified Application Counselor
