For more than 118 years, Backus Hospital has been the primary source of healthcare services for much of Eastern Connecticut. We are home to the only trauma center in New London and Windham counties, and are the only area hospital with LIFE STAR helicopter services. We deliver and coordinate a continuum of high-quality healthcare with the goal of improving the health of our communities.

JOB SUMMARY
The Care Management Liaison acts as the bridge between the hospital and extended care providers to ensure patients' care needs are met. The Care Management Liaison coordinates and executes the discharge planning process for patients requiring post hospital care and placement. The Care Management Liaison maintains ongoing communication with extended care providers, third party payors, as well as the patient and the patient's healthcare team to ensure a seamless transition of care.

Qualifications

Education: High school diploma or equivalent.
Experience: One year experience as administrative assistant, preferably in a healthcare setting.
