Overview

Develops cognitive rehabilitation programs and provides direct, consultative, and supportive cognitive rehabilitation  treatment and services to persons served in their home and essential community environments. Supervises  cognitive rehabilitation specialists.

Responsibilities

  • Develops and implements cognitive rehabilitation programs for individual persons served under the supervision  of a Neuropsychologist, in collaboration with the Trans-disciplinary team (IDT), and in accordance with  regulations, standards, and guidelines defined by the Society for Cognitive Rehabilitation, Division of  Developmental Disabilities (DDD), the Individual Program Plan (IPP), Health Insurance Portability &  Accountability Act (HIPAA), and established Bancroft policies and procedures.
  • Provides direct and consultative treatment services in cognitive rehabilitation to persons served

    o Effectively utilizes available information regarding therapy and treatment for each person served. Develops  cognitive rehabilitation and treatment-specific goals and objectives;  

    o Identifies and implements the design of functional strategies and the need for adaptive equipment that  enhances learning and creates an environment best suited to individual profile and needs.  o Provides family education and training in areas relevant to brain injury rehabilitation. o Assists persons served and family in identifying and accessing local community resources including allied  health professionals, adaptive equipment, medical supplies, and ancillary therapies where needed.  o Provides individual case management and utilization review services for assigned case load, including  written and oral communication with external case management and funding personnel.  o Makes recommendations for modifications to individual treatment plans where appropriate. o Provides transportation related to therapy for persons served during assigned sessions.  o Prompts and/or assists each person in the achievement of goals while actively encouraging the  independence and competency of each person within his/her ability limit; 

    o Plans and schedules frequency and duration of sessions in accordance with individual needs and service  plan guidelines.  

    o Provides suggestions regarding the utilization of techniques and/or materials to support and enhance  cognitive abilities; identifies barriers that may impede individual progress, and recommends environmental  accommodations to support functioning and facilitate the carryover of treatment goals; 

    o Demonstrates and instructs persons served and family in the proper use of functional and adaptive  equipment, devices and cognitive rehabilitation techniques;  

    o Utilizes a system of documentation that is professional, efficient, accountable, and conforms to federal, state  and other applicable policies and procedures; develops and maintains progress reports, session notes,  treatment summary reports, etc., as required by the IPP;

    o Provides training on a variety of topics and specific to cognitive rehabilitation to persons served, cognitive  remediation specialists, family members and other relevant persons regarding the implementation and  monitoring of treatment plans, etc. 

    o Supervises cognitive remediation specialists through direct observation of sessions, review of notes, review  of treatment goals, and case consultation. Working with Trans-Disciplinary team members, participates in diagnostic assessments of need for cognitive  remediation treatment and services and develops treatment plans for individual persons served:
  • Assists in determining the need for cognitive rehabilitation treatment for each person served; o Participates in the review of individual records, administration of standardized/formal/informal testing,  conducting of specialized screenings, etc., and provides recommendations for service delivery; o Participates with Admissions to conduct assessment of an applicant’s individual’s daily living activities in  their home or other community setting to determine capabilities, limitations, and rehabilitation needs; o Based on assessment, develops and modifies treatment plans based upon the individual’s abilities. As  requested by team members, provides reassessment of persons served, focusing on targeted areas of need  at established intervals;  

    o Plans and develops goals specific to cognitive remediation treatments that are achievable, functional,  measurable, and will be effectively utilized in each individual’s rehabilitation program.  

    o Attends TDT meetings, IPP meetings and rehabilitation conferences as needed.  

    o May oversee implementation of clinical home exercise programs.

  • Contributes in facilitating a team approach to program development, implementation, and monitoring by  effectively collaborating with all specialty/clinical disciplines, parents/guardians, etc., to identify and best meet the  individual rehabilitation needs of each person served.

  • Actively participates in team/departmental meetings, admission assessments, team conferences, family  meetings, etc., and, as requested, serves as a member of the Trans-Disciplinary Team (TDT) process in  conjunction with the Individual Program Plan (IPP). Attends and presents at Neurobehavioral rounds. 

  • Provides crisis intervention, as needed, through de-escalation techniques, staff support, and assistance. Follows  state-approved emergency procedures and utilizes Bancroft-approved crisis intervention techniques at all times. 

  • Maintains an awareness of current and changing trends, techniques, methods, and research in the field of  cognitive remediation treatments by attending learning workshops and professional seminars, reading journals,  attending in-service workshops, external conferences in area of specialty/clinical discipline, etc. 

  • Qualifications

    Education & Experience: 

    Master’s degree in Psychology, Education, Neuroscience or a related field required. Prior experience working with  neurologically-impaired individuals preferred. Must meet the certification criteria as defined by the Society for  Cognitive Rehabilitation. 

    Special Skills

    Ability to effectively communicate medical information, test results, diagnoses and/or proposed treatment in a manner  easily understood required. Knowledge of the administration, indications and contra-indications of various cognitive  remediation techniques and the ability to instruct individuals in the proper use of facilities and equipment required.  

    Ability to work independently in a field-based position, while effectively utilizing clinical and administrative resources  as needed. Demonstrates honesty, personal initiation, planning, and superior time management skills.  

    Effective communication, interpersonal, and organizational skills required with a strong desire to work with individuals  with acquired brain injuries. Ability to work independently and as an integral part of a team required. Ability to  implement crisis intervention techniques required. Attainment of CPR, First Aid, and Crisis Prevention and  Intervention certification within first 90 days of employment required. Valid driver’s license required in incumbent’s  legal name and current address with no provisional restrictions. Minimum 18 years of age required. 

    An incumbent in this position who has graduated from an accredited college with a Bachelors degree in  Occupational Therapy, Physical Therapy, Speech/Language Pathology, Social Work, Recreation, Art, Dance, Music,  Physical Education, Nutrition, Sociology, Special Education, Rehabilitation Counseling, Psychology or related Human  Services field of study AND possesses at least one year of experience working with individuals with developmental  disabilities or brain injuries may function as a Qualified Mental Retardation Professional (QMRP).
