Bay Path Rehabilitation and Nursing Center is currently seeking Full Time MDS Coordinator (40 hours/week). Our compassionate and experienced staff will help you reach your maximum potential in this role. Bay Path is a beautiful, 5-star facility located in Duxbury, MA. Requirements for MDS Coordinator:

  • Must possess valid unencumbered, LPN or RN State License and current CPR certification
  • Nursing degree from an accredited college or university
  • At least two (2) years of MDS experience preferred

Job Duties for MDS Coordinator:

  • Accurate completion of RAI for all Medicare patients in the facility
  • Provides completion of the nursing section of all Medicare MDSs and RAPs
  • Complete OBRA MDSs as PPS caseload allows
