Summary:
To serve as lead program coordinator for certain CMS/CMMI value-based programs including CPC (Comprehensive Primary Care Plus), PCF (Primary Care First), and PCMH (Patient Centered Medical Home) programs for the Central and Western Arkansas Regions. To lead program implementation and strategy for AHG contracted programs related to certain population health and value-based care programs. To engage with external stakeholders including providers, clinic staff, AHG staff, various Baptist Health entities, managed care organizations, and other vendors. To serve as lead value-based care analyst and lead data analyst to secure data, reports, and information necessary for successful performance in these programs.

Other information:
  • Bachelor’s degree in nursing, public health, healthcare administration or related field and 5 years full time professional experience in a healthcare setting or coordinating the implementation of health programs or Master’s degree in public health, healthcare administration, or related field and 2 years full time professional experience in a healthcare setting or coordinating the implementation of health programs

  • Strong organization skills, including the ability to prioritize multiple projects, work independently and collaboratively

  • Ability to work well with multiple teams and stakeholders

  • Excellent communication with presentations skills

  • Proficiency in software including Word, Excel, PowerPoint and Google Suite

  • Demonstrate time management skills and proven ability to meet deadlines

  • Manage multiple tasks simultaneously while completing work within allocated time frames

  • Knowledge of HEDIS measures and NCQA accreditation standards, preferred

  • Knowledge of health insurance, healthcare delivery, health policy, or value based payment, preferred

  • Ability to train individuals with health information technology, preferred Experience with Care Coordination roles and responsibilities in value-based care programs Experience with clinical practice transformation and/or change management principles

This job will be authorized 80.00 hours bi-weekly.
