Seeking Case Manager!Full-Time, Days, On-Site

Southern Indiana/Louisville, Kentucky area
This position leads the collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet patient' health needs through communication and use of available resources. The goal for this position is promotion of quality cost-effective outcomes including access to care and appropriate utilization of resources. A case manager is an expert and vital participant in the care coordination team who empowers people to understand and access quality, safe, and efficient health care services.
Position/Opportunity Highlights:
Qualifications:
- Registered Nurse with graduation from an accredited school
- Current licensure in the State of Indiana.
- Bachelor's degree preferred.
- Minimum of 3 years' experience in acute care nursing or Home Health Care
- Discharge planning/Case Management experience preferred
