What you'll doThe Manager Case Management Logistics Center will work Full Time, Days and is responsible for the recruiting, scheduling, onboarding, training, coordination, support, and evaluation of the Unit Clinical Care Coordinators, Logistics Center Clinical Care Coordinators (Logistics Center C3s) and supporting positions.
- The Manager Case Management Logistics Center assumes operational responsibility for patient progression, continuous process improvement, and the management of human, fiscal and material resources.
- The Manager Case Management Logistics Center will ensure that operational activities are consistent with and in support of the organization’s strategic plan.
- Maintains current knowledge of basic health care economics, trends and reimbursements methodologies and applies knowledge to daily practice.
- The Manager Case Management Logistics Center plans for and secures financial and human resources for the department.
- Oversees, supports and implements the goals of Care Coordination and the Logistics Center.
- Maintains oversight responsibility for patient flow and through-put of all acute patients.
- The Manager Case Management Logistics Center assists the Manager of Risk Management and Quality Improvement with process improvements, screening investigations and problem solving as needed.
- Responsible for educating staff on the implementation of clinical protocols. Participates in HMO contracts issues as needed.
- The Manager Case Management Logistics Center quality improvement activities and oversees the competencies within the department.
What we need from youEducation:
- Bachelor's degree in Nursing OR will complete Bachelor's degree in nursing within one year OR Bachelor's degree in Social Work required.
- Master degree in social work preferred.
Experience:
- 1-3 years of progressive, responsible healthcare supervisory experience.
License/Certifications:
- If you are a Nurse the following is required:
- Current NYS RN License.
- PRI and/or Screen Certification within 6 months of hire.
- If you are a Social Worker the following is required:
- NYS LMSW .
- Case Management/Social Work Certification within 6 months of hire.
