JOB TITLE: DISCHARGE COORDINATOR
REPORTS TO: SOCIAL SERVICES DIRECTOR
JOB SUMMARY:
Under the general direction of the Social Services Director, participates in providing a full range of social services designed to meet the needs of the residents. Serves as a resident advocate to assure residents’ rights are respected.
PRIMARY DUTIES AND RESPONSIBILITIES:
Discharges: Will be responsible for all aspects of discharge planning. Assess needs, coordinate and refer to appropriate agencies, coordinate with Assessment team as well as Nursing staff and Physician to ensure resident is educated on diet, medications, mobility and adaptive devices. Completes appropriate discharge forms and faxes referrals to appropriate agencies, ensuring that resident/family has a copy. Ensures proper equipment is obtained, coordinates equipment needs with Physician and DME supplier.
Communicates with Physician regarding Social Services and discharge planning needs of resident.
Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to residents when needed. Is able to obtain other resources as needed.
Consults with other departments as appropriate to collaborate on resident care and performance improvement activities.
Provides family tours when needed.
Completes Section Q of MDS.
Completes Discharge Care Plan.
Arranges and facilitates discharge planning meeting with resident and family members.
Makes community referrals to Region VII Agency on Aging, A&D Medicaid Waiver. Works with Medicaid Waiver Agencies Disability Network to identify residents with discharge potential to return to the community.
Attends weekly Medicare meetings.
Is knowledgeable of Resident Rights and ensures resident privacy and dignity and helps ensure a safe, secure environment for all residents.
Completes annual Inservice training requirements, maintains acceptable attendance, and dresses in accordance with Dress Code Policy.
Responsible for appropriate use of Facility supplies and equipment to minimize loss, waste and fraud.
Maintains confidentiality of all data, including resident, employee and Facility information.
Performs other duties as assigned.
JOB QUALIFICATIONS:
Social Services Technician registration; minimum of Associate’s Degree in Science, Arts or Business preferred or related field with minimum one year working under supervision of a licensed Social Worker
Marketing and long term care experience preferred, general knowledge of medical conditions and medications
Experience in data entry, Microsoft Word, Microsoft Excel preferred
Valid, unrestricted drivers license
Interpersonal skills necessary to work productively with all levels of healthcare personnel, effectively communicate with residents, families, and general public, filing and organizational skills.
Physical ability to sit in one position for extended periods of time.
Must be Covid Vaccinated.
