GENERAL SUMMARY:
Summary:
Provide assessment, diagnosis and treatment of behavioral, emotional and mental health issues that impacts the adolescents’ daily functioning. Develop treatment plans and conduct counseling that enables the adolescent to recognize and utilize internal and external resources to resolve their defined needs. Provide behavioral health education to adolescents and schools for mental, emotional and behavioral disorders within the High School Based Wellness Centers. Provide consultation within the school, community and for families of adolescents with mental health needs. Provide collaboration with and a conduit to Behavioral Health and support an integrated model of health care. Must be credentialed by Bayhealth Medical Center Medical Staff Ambulatory Department prior to counseling patients.
ESSENTIAL FUNCTIONS & ACCOUNTABILITIES:
1. Demonstrates the knowledge, skills and abilities necessary to provide psychological care, consisting of individual, group, family therapy and crisis interventions appropriate for the youth and young adult populations. This requires the ability to conduct assessments, diagnose, obtain and interpret pertinent clinical information, to identify age and culturally-specific patient needs and to provide the care needed.
2. To provide therapeutic interventions with the identified adolescent in order to enhance and foster their optimal functioning and interactions within the school, their home and community.
3. Demonstrates knowledge and proficiency in using and scoring various psychotherapeutic screening tools and evidenced-based clinical techniques including RAAPS, field standard testing for diagnosis and treatment purposes, including and not limited to, depression, anxiety, mood, ADHD, trauma, suicide and other screenings/assessments, Columbia Suicide Screen and other EPIC risk assessments to meet the needs of the youth and young adult population.
4. To continually assess the adolescent’s needs for appropriateness of treatment plan and make adjustments when necessary, including creating and updating treatment plans as decided by team or supervision.
5. Coordinates mental health services and transition of care to the community setting, as necessary. Maintains knowledge of resources including community providers and support organizations appropriate for patient population served.
6. Demonstrates the ability to provide highly-skilled behavioral health consultation to the clinical team.
7. Demonstrates quality clinical care and maintains documentation pursuant to the requirement of insurance and billing requirements using ICD-10 coding.
8. Collaborates with school administration, guidance, special education providers and other school personnel to evaluate and design an effective treatment plan for each student based on their cognitive abilities.
9. All other duties assigned within the scope and range of job responsibilities.

Required: Two years’ experience in a mental health setting with an adolescent population Preferred: Diagnostic Experience Experience with addictions counseling Experience with adolescents preferred
