What are the responsibilities and job description for the allcove Clinical Coordinator position at Beach Cities Health District?

GENERAL PURPOSE

Under the direction of the allcove Clinical Manager and Chief Programs Officer, the allcove Clinical Coordinator will be responsible for care management, clinical support and resource linkage to effectively meet the needs of young people ages 12-25 for the allcove Youth Wellness Center, while also ensuring youth voice in all stages of program development and implementation. 

 

ESSENTIAL FUNCTIONS

The duties listed below are intended only as illustrations of the various types of work that may be performed.  The omission of specific statements of duties does not exclude them from the position if the work is similar, related, or a logical assignment to the job.

 

CLINICAL DUTIES

  1. Intakes allcove clients, conducting in-depth assessments and reassessments, and identifies situations, capabilities and challenges to determine what services are required to meet client’s needs.
  2. Under the guidance of the allcove Clinical Manager and supervision of the Chief Programs Officer, provides clinical oversight and on-site mental health counseling and support to individuals, groups and families. Services may include counseling, workshops, supportive therapy groups and psychotherapy as needed.
  3. Formulates care plans, recommends and implements services, and follows up and monitors care plans. Provides updates on status of cases during bimonthly supervision with Chief Programs Officer.
  4. Identifies and develops support systems for clients; provides support to clients and caregivers; provides information and referrals either within allcove or through community linkages; conducts client advocacy when appropriate. Manages a caseload of assigned clients. Works collaboratively with other contracted clinicians and medical providers on-site.
  5. Maintains confidentiality and assures all participant and electronic health records are maintained accurately in datacove, a centralized data system for alcove, in compliance with HIPPA standards. Contributes data and information to datacove in accordance with the allcove core components and requirements. 

 

YOUTH ENGAGEMENT DUTIES

  1. Responsible for the development and implementation of allcove consistent with the youth-centric model and strategic plan.
  2. Responsible for recruiting, mentoring and leading the Youth Advisory Group and all associated workgroups in all activities including programming, scheduling, coordination, and staffing.
  3. Works collaboratively with BCHD communications department and allcove technical assistance provider to promote youth engagement and participation through marketing, outreach and community events.
  4. Develops partnerships with clinicians and organizations locally and supports the build out of a provider network to link allcove clients to community services and resources.          
  5. Provides education and training initiatives for staff and young people involved in youth participation and onboards all Peer Specialists.
  6. Serves on policy-making committees, assists in community development and supports community outreach efforts to bring awareness to allcove in the Beach Cities.
  7. Works collaboratively with BCHD’s Youth Services department to ensure integration of school and community-based programs and services and
  8. Responsible for oversight of the School Well-Being Line including but not limited to managing data collection, analysis and information & referrals.
  9. Exhibits clear understanding of the history, mission, and vision and serves as an ambassador of Beach Cities Health District.
  10. Serves as a role model for the District’s core values of compassion, integrity, accountability and excellence in actions and work product.
  11. Performs assigned training, duties, and work schedule as a disaster service worker on behalf of BCHD.
  12. Completes other projects as assigned.

QUALIFICATIONS GUIDELINES

Any combination equivalent to experience, education, and training that would provide the required knowledge, skills, and abilities would be qualifying.  A typical way to obtain the knowledge, skills, and abilities would be:

EDUCATION:

Master’s degree or post-secondary education in social work or marriage and family counseling. 

EXPERIENCE:

Requires three or more years prior experience in care management, clinical skills and volunteer supervision. Experience includes conducting client assessments, assessing for mental health concerns, formulating care plans, coordinating programs and managing client cases specifically for the youth clients aged 12 – 25 years old. Demonstrated understanding of the challenges and experiences of young people from diverse backgrounds including BIPOC, LGBTQ and unhoused young people. 

KNOWLEDGE:

Knowledge of clinical theory, human development, systems theories, strengths-based perspectives, normal and abnormal psychology, group process and counseling modalities.  Knowledge of law as related to mandated reporting, consumer protection (California Board of Behavioral Science policies) and the Tarasoff Law.   Knowledge of ethically based practice and limits of scope of practice and BCHD’s guidelines, polices, and procedures.   Broad knowledge of the range of services available to young people in the local community, health, education, employment and youth sectors.

ABILITIES/SKILLS:

Excellent working knowledge of the computer, including Microsoft Office programs (e.g., Word, Excel, Outlook, PowerPoint), and ability to learn software programs and databases. Bilingual Spanish-English skills are highly desirable.

SPECIAL REQUIREMENTS:

First Aid and Cardiopulmonary Resuscitation (CPR) methods; appropriate safety precautions and procedures; simple record keeping procedures.  Must be able to successfully pass background check, tuberculosis test and drug screen.  Must have a valid Class C California driver’s license and ability to maintain insurability under the District’s Employee Use of Automobiles Policy

Must have completed certification for Mental Health First Aid or demonstrate a willingness to complete training upon hiring.

Ability to work some evenings and weekends.

This position requires hearing, talking, and seeing. Specific vision abilities required by this job include close vision and the ability to adjust focus. While performing the duties of this job, the employee is regularly required to use hands to finger, handle, feel or operate objects, tools, or controls and reach with hands and arms. The employee uses finger dexterity in order to operate computer hardware for extended periods of time.  The employee is required to constantly sit and occasionally walk and stand to access counters, work areas and files; stoop, kneel, or crouch to access or place records or files; lift and carry records and documents, typically weighing less than 20 pounds.  

 

ENVIRONMENTAL ELEMENTS:

Employee works in an integrated youth health center for young people ages 12-25 and at community locations throughout the greater South Bay. Some evening and weekend work may be required.
