Registered Dietitian

Country Gardens Health and Rehab & Rehabilitation is seeking an Enthusiastic and Passionate Registered Dietician to join our team!

At Country Gardens Health & Rehabilitation, our residents are provided with a full range of skilled nursing and rehabilitative services to treat and support their needs in a safe and compassionate environment that fosters independence and dignity. To meet the needs of our community, our center features a rich spectrum of care that includes but not limited to Post-Acute Rehabilitation, Wound Care Management, Skilled Nursing, and Enhanced Alzheimer’s and Dementia Care.

This is a split position, 20hrs Country Gardens 20hrs Carvalho Grove in fall river

The Registered Dietician will be responsible for, but not limited to:

1. Coordinate nutritional care of residents and patients by completing nutritional assessments

2. Participate in care planning meetings as required

3. Provide nutritional counseling and education for residents and family

4. Meet DOH/state/federal and company standards 5. Perform duties independently without training or direction

Qualifications:

Position Requirements:

• Must possess Registered Dietician licensure within the state of operations
• High comfort level with enteral nutrition
• At least one year of experience in a long-term care setting
• Experience with MDS 3.0
• Strong oral and written communication skills
• Experience with electronic charting and the Nutrition Care Process preferred
• Familiarity with state survey process and compliance to nutrition care regulations
