POSITION SUMMARY

The Director of Community Health at BJHCHS is responsible for overseeing and coordinating all community health and outreach activities within the organization. This position requires a Bachelor's degree in Social Work, Public Health, or a related field, with a preference for candidates holding a Master's degree in the same fields and Community Health Worker certification.  The successful candidate will have a minimum of 3 years of experience in public health, community medicine, outreach, or the medical field. The Director will lead a team of Community Health Workers, Outreach Workers, and Social Workers, providing supervision and guidance. Applying a population health perspective, the Director will play a crucial role in improving the overall health and well-being of the communities served by BJHCHS.

 

RESPONSIBILITIES

  1. Leadership and Team Management:
    1. Supervise and provide leadership and mentorship to a team of Community Health Workers, Outreach Workers, and Social Workers.
      1. Develop and implement performance goals and objectives for the team and conduct regular performance evaluations.
      2. Provide guidance, support, and training to ensure the team's effectiveness and professional growth.
      3. Foster a collaborative and inclusive work environment that promotes teamwork and individual development.

 

  1. Community Health and Outreach Coordination:
    1. Develop and implement strategic plans for community health and outreach initiatives, in alignment with the organization's mission and goals.
      1. Coordinate and oversee all community health and outreach activities, ensuring their successful execution.
      2. Collaborate with internal departments and external partners to identify community health needs and develop programs to address those needs.

 

  1. Monitor and evaluate the effectiveness of community health and outreach programs and make recommendations for improvements.

 

  1. Program Development and Evaluation:
    1. Conduct needs assessments to identify gaps in community needs under the direction of the COO and other Senior leadership.
    2. Design, implement, and evaluate evidence-based programs and interventions to address identified needs.
      1. Ensure compliance with relevant regulations, guidelines, and funding requirements.
      2. Establish and maintain partnerships with community organizations, healthcare providers, and other stakeholders to enhance program effectiveness and sustainability.

 

  1. Data Management and Reporting:
    1. Develop and maintain systems for collecting, analyzing, and reporting data related to community health and outreach activities.
      1. Prepare reports on program outcomes, trends, and impact, and present findings to internal and external stakeholders.
      2. Utilize data to identify areas for improvement and guide decision-making processes.

 

  1. Budgeting and Resource Management:
    1. Participate in the development of annual budgets for community health and outreach programs.
      1. Monitor expenditures and ensure effective and efficient use of allocated resources.
        1. Seek additional funding opportunities through grants, partnerships, or other means to expand and enhance community health and outreach initiatives.

 

  1. Addressing Social Drivers of Health:
    1. Recognize and understand the impact of social determinants of health on community well-being, such as socioeconomic status, education, housing, and access to healthcare.
      1. Ensure that community health and outreach programs incorporate interventions that address the underlying social determinants of health.

 

  1. Community Engagement, Collaboration and Partnerships:
    1. Foster meaningful community engagement and participation in the planning, implementation, and evaluation of community health programs.
      1. Establish and maintain relationships with community organizations, leaders, and residents to gain insight into community needs and priorities.

 

  1. Actively participate in community coalitions, task forces, and committees focused on addressing community health needs.
    1. Seek opportunities for partnerships and joint initiatives to leverage resources, expertise, and reach maximum community impact.

 

  1. Health Education and Promotion:
    1. Develop and implement health education initiatives that empower individuals and communities to make informed decisions regarding their health.
      1. Coordinate and deliver educational workshops, seminars, and outreach activities that promote preventive care, healthy lifestyles, and disease management.
      2. Collaborate with the Operations team to develop culturally appropriate health promotion materials and campaigns.

 

The Director of Community Health and Outreach plays a vital role in promoting the health of the communities served by BJHCHS. By coordinating and leading community health initiatives, the Director will contribute to creating a healthier, more equitable community by addressing the underlying factors that impact health outcomes. This comprehensive approach will improve the overall well-being and quality of life for individuals and populations served by BJHCHS.

 

QUALIFICATIONS

  • Bachelor’s degree in social work, Public Health, or a related field (Master's degree and certification as CWH preferred).
  • Minimum of 3 years of experience in public health, community medicine, or outreach
  • Certification as a CHW and CHW Supervisor must be obtained within the first 6 months if not already completed
  • Strong leadership and management skills, with experience in supervising a diverse team.
  • Knowledge of population health principles and strategies.
  • Excellent communication, interpersonal, and collaboration skills.
  • Ability to develop and maintain partnerships with diverse stakeholders.
  • Proficiency in data management and analysis.
  • Understanding of relevant regulations and guidelines in community health and outreach.
  • Ability to adapt to changing priorities and work effectively in a fast-paced environment.
  • Commitment to promoting health equity and addressing health disparities.
