Accountability Objectives: The RN Case Manager position is responsible as a member of the interdisciplinary team for the assessment and care of patients in their residences/homes.

Minimum Requirements: Must be at least 18 years of age— graduate of an accredited nursing program. The RN Case Manager must be licensed as a Registered Nurse in the state of residence. Additional certifications/education may be required for specialty services to the requirements of regulatory agencies. His position is necessary to maintain CEUs per state regulations. Wo years of nursing experience may be preferred. Ome Health and Hospice experience is preferred. Comprehensive knowledge of general nursing theory and practice is required. Acceptable background check per regulations and company background check policy, valid driver’s license, personal auto insurance, and driving record, which allows candidates to be covered by agency insurance policy required. Also must possess all of the following: a sincere desire to provide service and support to persons in need; the ability to make sound judgments when given guidance and priorities; the ability to work as a team member to maintain consistency and quality in services; the ability to coordinate patient care services; and the ability to exchange support, constructive criticism and ideas with other employee members. Previous case management experience is preferred. In addition, the RN Case Manager must possess positive interpersonal skills and work cooperatively with co-workers and supervisors, demonstrating a supportive attitude.
