SUMMARY OF DUTIES:
The
Assistant Program Director of Children's Health Home Services is responsible
for supervising Senior Care Managers and Care Management staff. A Health Home
is a care management service model whereby all of an individual's caregivers
communicate with one another so that all of a patient's needs are addressed
in a comprehensive manner. This is done primarily through a "Care Manager"
who oversees and provides access to all of the services an individual needs
to assure that they receive everything necessary to stay healthy, out of the
emergency room and out of the hospital. Health records are shared among providers
so that services are not duplicated or neglected. Health Home services are
provided through a network of organizations-providers, health plans and
community-based organizations. When all the services are considered collectively
they become virtual "Health Home". The Assistant Program Director, with the
Program Director of Children's Health Home, will be responsible for hiring
and recruiting staff and enrolling eligible patients to join the Health Home,
developing a robust network of healthcare providers and achieving the
outcomes identified by the New York State Department of Health. The Assistant
Program Director of the Children's Health Home is the backup when the Program
Director of the Children's Health Home is not available.
RESPONSIBILITIES:
Supervise Senior Care
Managers and Care Management Staff:
Recruit and hire
quality Care Management staff
Ensure staff are
oriented and trained in the roles and responsibilities of Care Managers
Supervise and evaluate
the performance of the Senior Care Managers and Care Managers
Provide or arrange
necessary training for Care Coordinators
Train new staff in Netsmart
and other agency systems.
Children's Health Home
Program Responsibilities:
Supervise the Senior Care
Manager and Care Managers as needed
The Assistant Program
Director of the Children's Health Home is responsible for managing the
referrals and assigning them to the Care Managers
The Assistant Program
Director works with the managed care companies and community sources in
status of our referrals and in working together to help meet our client's
needs
Assistant Program
Director addresses client and staff concerns
Develops a Children's
Health Home policy binder to ensure that all Health Home policies by
Department of Health are adhered to.
Performs internal
chart audits to make sure that the case records are in compliance to the
regulations
Oversees the staff use
of the EMR in documenting patient care plans, all health home services and
tracking both patient progress and program outcomes.
Provide Health Home
Related Education
Provide education and
supervision to Care Managers as needed through in-services and small group
discussions
Provide education on
what a Health Home is internally and externally
Schedule and assigned
duties are subject to change based on the needs of the program and the
clients we serve.
REQUIRED KNOWLEDGE AND
SKILLS:
Knowledgeable of the treatment of
a broad range of psychiatric and physical illnesses
Awareness of Children's Health
Home best practices
Possess a strong healthcare/mental
health background
Knowledge and skill in clinical
and administrative supervision; ability to review care management records for
regulatory and policy compliance and for quality of care issues
Ability to work effectively in
groups, to team build an lead care management personnel toward achieving
agency goals
QUALIFICATIONS:
Masters degree
preferred in a healthcare professional (nurse, social worker, mental health
counselor, etc.)
Knowledge of and
experience with the treatment of both psychiatric and medical illnesses is
necessary as is knowledge of case management, care coordination and the
principals of person-centered care
A minimum of five
years' experience working in a Health Home and/or a New York State Office of
Mental Health licensed program and three years supervisory experience.
