As a Complex Medical Case Manager, you will be primarily responsible for providing medical case management services to designated refugee clients with complex medical and mental health issues.

This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements.

ESSENTIAL JOB RESPONSIBILITIES

  • Provide case management services to clients designated for these intensive medical services;
  • Assist the resettlement team in preparing for arrival of complex medical and mental health cases;
  • Complete intakes and assessments of new cases in accordance with program and agency expectations;
  • Maintain clear and up-to-date on case notes and other required documentation;
  • Create and coordinate referrals to local community services based on client’s need(s);
  • Coordinate the appropriate referrals to mental health services;
  • Assist clients in applying for Social Security Income, guardianship, home health care, and other life sustaining items;
  • Communicate scheduling needs and follow-up steps to medical support staff;
  • Assess cases for improvement, areas of growth, and possibility case closure on a regular basis, while documenting the items accordingly;
  • Assist clients in achieving self-sufficiency;
  • Complete paperwork within appropriate time frames;
  • Actively participate in weekly case management and supervision meetings;
  • Attend other medical related planning meetings;
  • Identify clients for nursing students to work with and facilitate the introduction;
  • Assist in developing and facilitating refugee health educational classes and facilitate psychoeducation groups;
  • Assist in creating and/or adapting culturally appropriate health education and resource guides;
  • Serve as medical resource to other members of the team, while also assisting with developing and coordinating trainings for staff and volunteers on medical and safety topics;
  • Develop and maintain an understanding of current refugee issues through personal reading, attendance at related seminars, participation in related task forces and committees;
  • Transport clients on an as needed basis;
  • Stay abreast of all agency, federal, and state regulatory requirements related to social services;
  • Complete other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s level degree in Human Services, Social Work or related field of study from an accredited college; Master’s level degree in Human Services, Social Work or related field of study from an accredited college, preferred;
  • At least one (1) year of experience providing medical case management;
  • Bi-lingual: English/Spanish, Arabic, Burmese, Farsi, French, Kinyarwanda, Kirundi, Nepali, Somali, or Swahili;
  • Prior work experience with serving refugees, preferred;
  • Mature and stable judgment as well as sensitivity to various cultures and the unique history of refugees;
  • Excellent verbal and written communication skills;
  • Demonstrated clinical, therapeutic, and crisis intervention skills;
  • Ability to work independently and exercise a high level of confidentiality;
  • Must be willing and able to work some evenings and weekends when necessary;
  • Computer and typing skills sufficient to perform essential job functions;
  • Must be 21 years old with a valid driver’s license with at least 3 years driving experience to operate a vehicle on behalf of Bethany. Must also pass a Motor Vehicle Records (MVR) check and maintain a reliable vehicle with proof of adequate insurance coverage;
  • Pass a criminal history screen, including state and local child protection agency registries;
