Position Summary
The MDS coordinator assesses capabilities of residents and compiles information needed for initial MDS, MDS quarterly assessment, and written care plan.
Duties/Responsibilities
1. Maintains certification to give Mantoux tests, CPR, and first aid.
2. Writes and/or assists the director of nursing in writing resident care plans for each resident. Chairs the care plan meetings.
3. Works as a charge nurse on the floor when required.
4. Assists in AIMS testing.
5. Updates MDS care plan schedules.
6. Completes computerized or written MDS forms and quarterly summaries within required time frame. Assures completion of the MDS by each department within the required time frame. Update care plans as needed.
7. Transmits completed MDS forms to appropriate state and Federal agencies at required time frame, as required by regulation.
8. Keeps Medicare certifications up to date.
9. Informs residents/responsible parties of Medicare covered days and notifies them of their last covered day.
10. Helps maintain communication about Med A patients between therapy staff and Hoosier Village staff; (e.g., new admissions; admit and discharge dates; RUGS; care plan schedules; and occupational therapy, physical therapy, and speech therapy patient treatment times).
11. Handles problems with difficult or disturbed residents, family or sponsor/surrogate.
12. Teamwork and participation: maintains an effective relationship with all departments and professional staff; and participates on facility committees as assigned.
13. Participate in the on-call nursing rotation.
14. Reporting and response:
a. Reports verbally or in writing unusual resident behavior to supervisory personnel.
b. Reports verbally or in writing concerns expressed by family members, visitors, or other persons visiting the facility to supervisory personnel.
c. Communicates, either verbally or in writing, any problems or concerns to his/her supervisor.
d. Reports all equipment malfunctions/breakdowns and other maintenance needs to supervisory personnel.
e. Responds to emergency situations effectively and efficiently and, in case of an emergency, provides direct assistance to residents in evacuating building.
15. Training requirements:
a. Attends required in-service meetings and departmental staff meetings and completes any written, verbal, or visual requirements.
b. Continually updates knowledge of current medical trends, through medical journals/literature, new developments in the nursing field, and state and federal regulations, especially as they pertain to geriatric nursing.
c. Receives knowledge of and demonstrates the ability to perform all emergency procedures including fire & safety, tornado, and disaster evacuation and participates in all subsequent emergency preparedness training exercises.
d. Completes annual in-services according to schedule.
e. Maintains knowledge and training in exposure control plan and universal precautions.
f. Attends continuing educational workshops and seminars at the discretion of the director of nursing and/or the health center administrator.
16. Performs other duties as assigned by the director of nursing and/or the health center administrator.
Qualifications
Must have a license to practice in the State of Indiana as R.N. Must have at least two years of experience in geriatrics. Additionally, the employee must demonstrate the following: ability to communicate effectively, both verbally and in writing; basic computer related skills; and other skills as required.
As a condition of employment, Hoosier Village requires that all new hires provide evidence of full or vaccination against COVID-19 within 3 days of employment. If a new hire wishes to submit a request to be exempt from the mandatory COVID-19 vaccination policy due to a medical condition, sincerely held religious belief or other legal requirement, the associate will have three days after an offer of employment has been accepted to submit their exemption request for review and consideration by Hoosier Village.
