Role Purpose
Responsible for assessing and coordinating the physical, psychosocial and spiritual needs of the patient and family at end-of-life, as well as pain and symptom management at all stages of the chronic disease process. Develops and coordinates the palliative care plan with physicians and nurses, along with services such as Care Coordination, Pastoral Care, Home Health and Hospice care.
Responsibilities
- Consults with physicians, patients, families and the Palliative Care Team to develop and modify individualized plans of care.
- Provides ongoing case management for seriously ill and end of life patients.
- Provides educational opportunities for physicians, nurses, and other allied health care providers regarding palliative care and advanced directives, and integrates palliative care topics into community education programs.
- BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
Minimum Requirements
- Associate's Degree- Nursing
- 2-5 years experience
- Registered Nurse (RN) license

- Bachelor's Degree- Nursing
