What are the responsibilities and job description for the Care Coach - Care Coordination position at BJC?
- Full-Time; benefits eligible!
- Primarily Monday - Friday 8am - 4:30pm; also to include 1-2 weekend shifts per month. Weekend shifts would be in exchange for a weekday shift.
- Work from home position, but must live in the St. Louis metro area to come in for training and the occassional onsite meeting.
- This individual will perform a high volume of outpatient calls to patients who have had recent Emergency Room visits or inpatient admission.
- Ideal candidate will be comfortable with Epic and Teams, and have previous Healthcare experience (preferrably in a Primary Care or Specialist clinic).

The Care Coach, in collaboration with the interdisciplinary care team, is responsible for helping facilitate patient care along the continuum through effective transition planning. This position assists patients with achieving optimal health, access to care and appropriate utilization of resources to ensure patients receive the right care at the right time.
