What are the responsibilities and job description for the Case Manager- FAR ROCKAWAY position at Black Veterans For Social Justice Inc?
Using DHS's CARES database monitors residents services, assesses, provides statistical reports on a weekly and monthly bases as required by DHS, NYSDSS and the assessment shelter program.
Duties and Responsibilities:
Initiates face to face interview with the residents upon admission to the facility, complete all intake forms as required by DHS, NYSDSS and assessment program.
Ability to summarize, evaluate and analyze data and information from case-work ups and other documentation as required
Ability to provide an initial situation and diagnostic impression regarding clients needs for each client on caseload.
Maintain a caseload of 25-30 clients
Develop a weekly service plan, complete progress notes for each case session.
Assist residents who have been improperly referred for relocation.
Assist housing counselor with pertinent demographic data to facilitate residents to move to independent living
Update CARES to reflect the accuracy of client history
Attend weekly Social service meetings and/or mandatory staff meetings.

Minimum Requirements:
Bachelors Degree in social work with experience in a program serving the homeless or a similar program
High School Diploma or equivalent with FOUR (4) years experience in a residential program serving the homeless or similar population
DHS CARES experience
Bilingual a Plus
