BLC Home Health, Inc. is currently seeking a compassionate and dedicated Medical Social Worker (MSW) to join our team in providing exceptional care to our clients in the comfort of their homes. As a Medical Social Worker in our home health division, you will play a vital role in assessing patients' psychosocial needs and connecting them with resources to improve their overall well-being and quality of life.
Responsibilities:
- Conduct comprehensive psychosocial assessments to identify patients' social, emotional, and financial needs.
- Develop individualized care plans in collaboration with patients, families, and the interdisciplinary team to address identified needs and goals.
- Provide supportive counseling to patients and families coping with illness, disability, or end-of-life care.
- Advocate for patients' rights and assist them in accessing community resources, financial assistance programs, and support services.
- Collaborate with healthcare professionals, including nurses, physicians, therapists, and community agencies, to ensure holistic and coordinated care.
- Document all assessments, interventions, and patient interactions accurately and in a timely manner in accordance with regulatory standards and agency policies.
Requirements:
- Master's degree in Social Work from an accredited program.
- Current license or certification as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the state of California.
- Minimum of one year of experience as a Medical Social Worker, preferably in home health, hospice, or a related healthcare setting.
- Strong clinical assessment and counseling skills with the ability to provide empathetic and culturally sensitive care.
- Excellent communication and interpersonal skills.
- Reliable transportation and a valid driver's license.
- Ability to work independently and as part of a multidisciplinary team.
