Bluegrass Transitions Care- Irvine, KY
Coming home from the hospital or facing chronic health conditions can be challenging. It is our goal as a Transitions Care team member to support our patients and help prevent a return to the hospital. As a Bluegrass Transitions Care team member, you will work directly with patients and their caregivers, providing education and support in patient homes and community settings, to help patients better manage their chronic health conditions, and reduce unnecessary hospitalizations.

Bluegrass Care Navigators Offers
The Clinical Care Coach will be responsible for providing coaching services to eligible patients in various environments and locations. The staff will develop a wellness coaching relationship with clients and assist the client towards better health by providing support, encouragement, education, and resources when needed.

The Clinical Care Coach will be responsible for providing coaching services to eligible patients in various environments and locations. The staff will develop a wellness coaching relationship with clients and assist the client towards better health by providing support, encouragement, education, and resources when needed.
Requirements
- Is a graduate of an accredited school of professional (registered) nurses, or practical nurse or vocational nursing program.
- Is currently licensed as an RN or LPN through the State Board of Nursing.
- Minimum of 1-2 years’ experience in home health, health care or community-based organization.
- Strong communication and organizational skills.
- Knowledge of medical terminology.
- Strong education/teaching background.
- Ability to be flexible and adjust practice based on client population being served.
- Experience in problem solving, managing complex problem, and developing solutions.
- Strong customer service skills and the ability to work independently.
- Ability to exercise judgment and discretion in confidential matters.
- Must be able to make home visits to patients in a variety of settings; mobile homes, apartments, homes, nursing facilities and condominiums that may not have elevators.
- Must be able to drive for extended periods of time with no ill effects.
- Must be flexible, good written and oral communication skills.
- Must have computer access (PRN only) and excellent computer skills (ALL).
- Must have the ability to give and receive constructive criticism.
- Employees will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
- Must be a licensed driver with automobile that is insured in accordance with state and/or BCN requirements and is in good working order.
