Job Title: Clinical Social Worker, LCSW

Job Summary:

The Project Clinical Social Worker (CSW) will conduct comprehensive assessment in person and via telehealth using standardized screening and assessment instruments for poly SUD, co-occurring mental health disorders, and exposure to domestic violence.

Duties & Responsibilities:

Responsibilities include, but are not limited to;

· Standardized brief screens to be used may include MAST (Michigan Alcohol Screening Test), AUDIT (Alcohol Use Disorders Identification Test), MMS (Mini Modified Mental Health Screen), BDI (Beck Depression Inventory), TSQ - (Trauma Screening Tool), and LEC - 5 (Life Events Trauma Check List).

· Screen for domestic violence with DVSAT (Domestic Violence Safety Assessment Tool). Standardized longer form instruments we use are: DAST (Drug Abuse Screening Test - long version), Center for Epidemiological Studies

- Depression Scale, SIAS (Social Interaction Anxiety Scale), PCL (PTSD Checklist

- civilian version), PHQ-9 (Patient Health Questionnaire for depression), and MDQ (Mood Disorder Questionnaire).

· Based upon in-depth assessment, the CSW will provide clients with individualized service planning staged upon client needs and client goals, offer clients choices, and provide access to support services (Recovery Support Services, HIV/HCV, medical, insurance enrollment, patient navigation, employment, education, housing) and linkage to a comprehensive range of evidence-based behavioral health services provided at Argus Community or another agency of client preference.

· The CSW will regularly meet with client’s in-person or virtually to review adherence to treatment plans and develop adjustments to treatment plans if needed.

· The CSW will provide tobacco cessation counseling and treatment service referral. The CSW will conduct GPRA interviews at baseline, 6-months, and discharge.

· Participates in twice-monthly program Performance Management meetings. Two years’ experience providing services for populations affected by substance use disorder and co-occurring mental health disorders.

· The CSW will have received training in the EBPs of Motivational Interviewing, Stages of Change, and Harm Reduction Theory, and in the provision of trauma-informed services.

· NYS Mandated Reporter for Child Abuse and Neglect.

· Bilingual preferred.

Education/Criteria:

· NYS LCSW.
