What are the responsibilities and job description for the Social Worker II - Healthy Baby Healthy Child - CAFH position at Boston Public Health Commission?
Healthy Baby Healthy Child is a city-funded home visiting program including registered nurses, paraprofessional public health advocates and mental health clinicians who provide skilled nursing, education, case management, and mental health counseling to pregnant women and parenting families of children up to the age of 5years.
Works as a member of a multi-disciplinary team in assessing, developing, implementing and evaluating the clinical component of the individualized service plan for each client. Meets regularly with Director of Behavioral Health for standards of practice, supervision, consultation and evaluation. Performs clinical duties according to social work and program standards. Provides clinical care according to identified client and community needs. Supervises assigned Public Health Advocates and participates with the division leadership team in program development and implementation. Takes a leadership role in coordination of special projects. Provides in home psychotherapy to assigned families and works collaboratively with the providers at primary care sites through participation in case conferences and in other informal communications. Facilitates education and training for clients during home visits, clinical site appointments or in group settings in the community. Develops intervention strategies for each client. Delivers services and documents clinical interventions according to social work/counseling standards and program protocols. Identifies emerging problems and solutions for clients and their communities. Continues professional education to enhance knowledge, skill and ability to maintain a high level of clinical practice and maintain licensure.
Applies family systems, psychosocial theories, adult learning and knowledge of the effects of substance abuse and domestic violence to the family to determine family assessment criteria, measurable objectives and relevant interventions with families in home and community settings.
Offers support and information about family counseling and a variety of family support services as needed to team members and other community service providers who work with enrolled children and families and may include other family members, emergency shelters, food pantries, counseling services, substance abuse treatment programs, multi service community agencies and information about interfacing with agencies offering counseling or social services to families as needed. Establishes and maintains a current file of community resources for families and shares information with families and community providers.
May assist the Healthy Start Systems division director in updating family intake and assessment materials to reflect changing family and community needs.
Keeps informed and shares information about parenting skills, support and advocacy and assists the program to acquire and use these materials.
Performs other duties as required.
Masters prepared professional counselor or Social Worker eligible for licensure in Massachusetts (LCSW, LICSW, LMHC, etc)
MCH Experience in family systems and care of families of young children and in multi cultural settings.
Knowledge of Boston community and community resources.
Previous community health experience including home visiting.
Excellent writing and oral communication skills with families and staff.
Bilingual fluency preferred
