What are the responsibilities and job description for the Harm Reduction Coordinator position at Breaking Ground?
Reporting to the Senior Director, Evidence-Based & Best Practices, the Harm Reduction Coordinator has a primary role in developing and implementing Harm Reduction programming across the social services portfolio, including Street Outreach, Transitional Housing, and Permanent Housing. The position will provide clinical oversight of site-based Harm Reduction Specialists (currently 3, with potential for additional Harm Reduction Specialists and/or Peer Specialists in the future; administrative oversight will remain at the site level). The Harm Reduction Coordinator is responsible for leading the team of Harm Reduction Coordinators in the development and standardization of a harm reduction toolkit and best practice model for scalable, evidence-based/evidence-informed, person-centered harm reduction programming across the portfolio. The toolkit will establish a standardized, baseline standard for providing harm reduction programming across the portfolio and will include, at minimum, expected programming (e.g., groups, trainings, etc.), best practices in hospital coordination for substance use-related hospitalizations/emergency room visits, standardized safety plan and consumption assessment templates.
This position's schedule will be primarily Monday - Friday, 9:00am - 5:00pm with some evenings and weekends as needed to provide programming support. The position will be expected to have a minimum of 3 days on site with up to 2 days remote work as programming permits.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Lead development and implementation planning for a written, standardized, scalable, person-centered harm reduction toolkit for use across Breaking Ground’s social services portfolio
Provide clinical oversight of a small and growing group of site-based Harm Reduction Specialists with potential for addition of Peer Specialists in the future
Partner with site-based Harm Reduction Specialists to provide individual and/or group harm reduction programming
Collaborate with Senior Director, Evidence-Based & Best Practices and Coordinator, Staff Development & Client experience to develop and deliver training on harm Reduction practices
Act as Program Director for Breaking Ground’s New York State Opioid Overdose Prevention Program (OOPP) including completing monthly reporting on Naloxone distribution and providing training in opioid overdose prevention to staff and clients
Partner with Facility & Operations staff to support training and education around needlestick prevention and sharps container distribution and maintenance
Partner with Program Development Coordinator to develop and foster relationships with harm reduction resources and programs
Liaise with site-based and administrative medical providers to promote access to medications for addiction treatment (MAT), syringe access, and other harm reduction modalities
Partner with Senior Director, site-based staff, and Breaking Ground data services team to develop data collection and reporting tools to track harm reduction services, outcomes, referrals, and substance use-related health care utilization
Perform other related duties as assigned
MINIMUM QUALIFICATIONS:
MSW, similar credential, or extensive equivalent experience required. LMSW or LCSW preferred
In-depth knowledge of harm reduction practices, including working knowledge of evidence base for harm reduction practices and commitment to remaining up to date on existing and developing best practice models/concepts
Proficiency in Motivational Interviewing practices
Excellent computer skills, including proficiency in Office 365 and electronic case management or health record systems (AWARDS experience preferred)
Bilingual Spanish/English is preferred
