The leader in in-home care services is looking for Behavioral Case Managers to join its growing team! In this role, you will partner with the Clinical Director, Clinical Care Manager, and the healthcare team to administer and coordinate skilled nursing care to mental health patients requiring home care services. You will provide direct patient care and case management, complete timely documentation, and communicate with physicians and other services. Responsibilities:
- Work one-on-one with patients outside of a clinical setting.
- Support and coordinate mental and physical healthcare of patients on an assigned caseload.
- Develop, implement, monitor, and evaluate patients’ care plans.
- Provide ongoing education and support to patients and families.
- Administer medications as needed or directed.
Qualifications:
- An ADN is required, and a BSN is preferred.
- Active RN license.
- 1 years of RN experience in behavioral health.
- Valid driver’s license.
Additional info:
- Full-time, permanent position
- Multiple shifts available; Monday-Friday; 7a-3p, Monday-Friday; 12p-8p, and Saturday-Sunday; 7a-12p and 4p-8p
- Pay; $74,000-$80,000 base bonuses.
