- A licensed SW with a master's degree in social work.
- Experienced in counseling and managing casework needs of the terminally ill is a plus
Responsibilities
The social worker shall assist the patient and family in understanding social and emotional factors related to the patient’s health and shall include, but not be limited to:
- assessment of social and emotional factors that impact the patient’s health status
- assist in the formulation of the plan of care
- provide services within the scope of practice, as defined by state law, and in accordance with the plan of care
- coordination with other interdisciplinary team members and participate in IDG conferences
- prepare clinical and/or progress notes for incorporation into clinical record
- participate in discharge planning, and in-service programs related to the needs of the patient
