JOB SUMMARY: Under the direct supervision of the Program Manager, the Navigator will be responsible for outreach, recruitment, HIV, STI & HCV testing for the Substance Abuse and HIV Prevention Navigation grant. The Navigator will provide linkages and navigation services, including accompaniment, follow-up services, and support for newly HIV-diagnosed clients or out-of-care people living with HIV/AIDS (PLWH). In addition, assist with administering Evidence-Based Interventions (EBIs) via group settings on the misused substance use and HIV.
DUTIES AND RESPONSIBILITIES
- Formalize partnerships through a memorandum of understanding (MOU) with partnering agencies. Deliverable: 6 MOUs
- Recruit providers in the community: (two of which must be youth service agencies for collaboration, training, and education relating to the issue of substance misuse and HIV). Formalize partnerships through a memorandum of understanding (MOU) with partnering agencies.
- Outreach activities: Research venues to conduct outreach to the population of focus (POF).
- Assist with outreach activities to recruit the population of focus (POF) through community outreach, health fairs, and tabling events. Deliverable: Reach 200 people annually
- Attend weekly outreach and social media meetings
- Gather data from youth focus groups annually with the POF to gather feedback and input on potential campaign and message Deliverable: 2 focus groups.
- Assist with screening and testing the POF for HIV, STD,s & HCV, (Conducting Behavioral Risk Assessments and MSSI screening tools)
- Document HIV, STD & HCV results following all procedures following DOH guidelines for specimen handling Deliverable: HIV- 75 annually, STI – 75 annually, HCV- 75 annually
- Facilitate evidence-based intervention (Contingency Management with Life Skills, Too Good for Drugs and Violence, and SPORT) groups at youth services organizations. Assist with soliciting feedback from POF through pre-and post-test data for EBI’s. Deliverable: 24 groups with 240 program participants
Navigation and referral services:
- Develop Action Plans to address needs identified in the behavioral risk assessment.
- Assist with implementing navigation services (accompaniment, follow-up services, etc.) for newly HIV-diagnosed clients or out-of-care PLWH as well as navigation to substance misuse and other services as needed.
- Conduct detailed Behavioral Health Risk Assessment of clients’ health risk and HIV status to determine a client’s risk of acquisition of HIV and STD/HCV.
- Conduct linkage and navigation activities to connect the clients to medical care, benefits/entitlements/ food services, scheduling reminders, transportation/escort to appointments, addressing food insecurity, benefits/entitlement enrollment assistance, CD4/Viral Load monitoring, etc.
- Serve as a back-up driver for the agency mobile unit
- Performs other duties as assigned based-off program needs.
EDUCATION:
- MINIMUM: HS/GED and at least five years’ experience with Substance Abuse and HIV/AIDS issues.
- Bachelors Degree from an accredited college in psychology or human services or relative subject.
WORK EXPERIENCES:
- MINIMUM: Two (2) years’ experiences administrating HIV testing
- One (1) year experience with outreach and creating partnerships.
- Five (5) years experience with substance Abuse and HIV/AIDS issues preferred
- Experience providing Navigation Services
- Excellent with public speaking
- Experience delivering Evidence-based intervention, a plus
- Experience in data collection and analysis
- Knowledge of motivational interviewing techniques and addiction counseling
LICENSURE:
- CASAC-T/ CASAC
- A valid New York State driver’s license with at least 2 years’ clean motor vehicle record.
