Overview
As a Care Coordinator, you will act in a liaison role with clients to ensure appropriate care is accessed as well as to provide the necessary community resources. You will work in a team with other professionals and spend the majority of your time in the community engaging directly with clients. During the COVID-19 pandemic, most work will be through Telehealth and phone.
In this role, you will work with medically complex patients who may be experiencing significant social barriers or behavioral health conditions.
Primary Responsibilities:
- Create a positive experience and relationship with the client
- Help the client set goals and develop a care plan to achieve those goals
- Develop a Health Action Plan and ongoing progress notes
- Proactively engage the client to manage their own health and healthcare
- Support the client to improve their well-being by staying out of the hospital and attending regular visits to their primary physician
- Help the client engage with mental health and substance use treatment in the community
- Support the client after hospital admissions
- Provide the client with education on community resources and benefits
- Conduct post-discharge activities from the hospital facility and support connection to social services
- Support the client to engage in work or volunteer activities, if desired, and develop stronger social supports through deeper connections with friends, family, and their community
- Partner with the care team (community, providers, internal staff)
- May conduct Health Risk Assessments (HRA) if needed
- Conduct outreach through a phone call to engage clients
Required Qualifications:
- Must have a reliable means of transportation
- Valid Washington State driver's license
- Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area.
- Ability to create, copy, edit, send and save documents, spreadsheets, and correspondence in Microsoft Office, including Microsoft Word, Microsoft Excel, and Microsoft Outlook
- Ability to travel locally within the county up to 60% of the time
- Must be able to read and write English
- Must have home internet
- Must be able to document in the electronic platform
Preferred Qualifications:
- Bilingual
Soft Skills:
- Ability to work independently in a virtual environment and Hybrid environment
- Interest in Behavioral Health
- Previous experience as a Behavioral Health Technician or in Case Management Role
Compensation:
27$/hr with an opportunity to make 30$/hr. We also provide great work-life balance and other benefits.

- Current license as registered nurses, advanced registered nurse practitioners, practical nurses, psychiatric nurses, psychiatrists, physician assistants, clinical psychologists, mental health counselors, agency affiliated counselors, marriage and family therapists or certified chemical dependency professionals, or;
- Certified Medical Assistants with an Associate Degree or Indian Health Services (IHS) Certified Community Health
