Community Health Worker Job Description

Bright Star Community Outreach is seeking a dedicated Community Health Worker to serve as a liaison between our organization and the community, fostering trust and building relationships with residents and families in Chicago's South Side. The ideal candidate will focus on addressing social determinants of health and implementing strategies to improve overall well-being.

Responsibilities:

• Conduct outreach activities at predetermined events and locations to engage and recruit potential participants.
• Develop and implement recruitment strategies in alignment with organizational goals and sponsor/funder agreements.
• Educate participants about program specifics and benefits.
• Collaborate with team to schedule pre-screening visits for interested individuals.
• Cultivate positive and supportive relationships with participants.
• Maintain open communication with the Team Lead to address any performance or enrollment issues.
• Stay informed about community resources, services, and programs to better assist participants.
• Establish connections within the community and foster positive relationships with stakeholders in the community.
• May involve walking and direct interaction with community members.

Qualifications:

Technical Skills:

·High school diploma or equivalent is required.

· Previous experience in community outreach or related field is preferred.

·Strong interpersonal skills and ability to build rapport with diverse populations.

·Excellent communication and organizational abilities.

·Knowledge of social determinants of health and their impact on community well-being is preferred.

·Comfortable working independently and as part of a team.

·Proficiency in Microsoft Office suite and basic computer skills.

·Flexibility to work a hybrid schedule, with a minimum of 20 hours per week.
