On an average day as a PRN Chaplain (referred to internally as a Spiritual Care Clinician) you will:
- You will have the opportunity to collaborate with our clinical teams (physicians, social workers, nurses, therapists) to contribute to that mission. These passionate team members will depend on you to assess and support the spiritual and bereavement needs of patients and their families.Offer short-term individual counseling, crisis intervention
- Develop an individual plan for spiritual care in conjunction with the hospice interdisciplinary team, and when necessary in coordination with the patients/families/caregiver, church, clergy or spiritual family in order to accomplish a holistic plan of care for each patient/family/caregiver.
- Conduct a personal ministry to patients/families when other resources are not available or appropriate, including conducting funerals and memorial services when requested
- Document all contact with patient/families, including spiritual assessment, home visits and important telephone calls in relation to the Plan of Care in the patients clinical record.
- Be available to other care team members for bereavement support
- And perform additional, related duties as needed

Desired Skills/Qualifications
- Willingness to work a PRN/Part-Time schedule
- Passion for providing the best possible care to patients and their families
- Excitement for doing work that makes a difference while enjoying the perks of great work/life balance
- Have a valid driver's license, auto insurance, and reliable transportation for the purpose of traveling to our patients
- At least one year of hospice experience
- Clinical Pastoral Education Training and/or Spiritual Care Counseling Training preferred
- Comfortable in an interfaith setting
- Are able to demonstrate the level of knowledge and skills normally associated with a bachelors degree, including knowledge of Hospice, death, dying, grief and bereavement
