As the Social Worker, you will have the opportunity to collaborate with The Director of Clinical Services and other clinical team members (physicians, chaplains, therapists) to contribute to that mission. These passionate team members will depend on you to provide social services to patients and their families and to be a reliable go-to problem solver.

On an average day, you will…
- Offer short-term individual counseling, crisis intervention
- Assist in providing information and preparation of advance directives
- Assist with funeral planning issues and transfer of responsibilities regarding fiscal, legal and health care decisions
- Identify support systems available to reduce stress and facilitate coping with end-of-life care
- Identify and utilize appropriate community resources and assessing patient/family ability to access them
- And perform additional, related duties as needed
Qualifications

- Have at least 1 year of hospice work experience
- Have a Master’s degree in Social work from a school accredited by the Council on Social Work Education
- Have a valid license with the state you're applying to
